Our names are Dayle Hawes and Stephanie McCall, and we are Consumer Advocates at SIILC.

We are privileged to collaborate with the Independent Living Center in Richmond to provide Chapter 2 Vision Services in our six-county service area. Our program is designed for individuals aged 55 and older who experience significant vision loss that cannot be corrected with basic optical lenses. This includes conditions such as blindness, macular degeneration, cataracts, glaucoma, and diabetic retinopathy, among others.

Some of the items we provide at no cost include talking clocks, talking watches, handheld lighted magnifiers, large print calendars, cooking aids, talking blood pressure monitors, bump dots, and even fun items like playing cards and bingo cards. These resources help our consumers enjoy living independently.

We also offer CCTV devices, Ruby magnifiers, and OrCam readers, which can be loaned to our consumers.

Throughout the year, we conduct presentations at senior meal sites, senior living facilities, clubs, and organizations. This allows us to reach a large number of people in a short time and promotes our Center's other services.

In addition to the Vision Program, we assist in other areas as well. We provide assistive technology items for loan, including wheelchairs, walkers, rollators, bedside commodes, shower chairs, and canes, available for both short-term and long-term use.

We also offer information and resource services, connecting individuals with local resources for housing, transportation, food pantries, job training, education, communication services, insurance, and much more.

My name is Christine Sass and I am the Home Care Coordinator here at SIILC.

SIILC’s personal care attendants provide supportive home-based alternatives to nursing facility care. Our local, reliable, non-medical services include Homemaking, Attendant Care, and Respite services.

Some of our Homemaking services include house cleaning (sweeping, mopping, dusting, dishes), laundry, trash removal, running errands, meal preparation, and grocery shopping.

Our Attendant Care services are available to people with disabilities of all ages and for older adults. We are able to assist with bathing, grooming, dressing, meal preparation, transferring, reminders to take medication, and escorting clients to doctor’s appointments.

Respite services can also be provided in the absence of the usual caregiver.

Our programs and services are dedicated to helping people remain in their homes, maintain independence, and/or return to community living from nursing facilities.

My name is Mhelody Martinez and I am the Area 12 Long Term Care Ombudsman.

My mission is to advocate for residents of long-term care facilities, which includes nursing facilities and licensed assisted living facilities. My primary purpose is to promote and protect the Resident Rights guaranteed to them under federal and state laws. Facilities are also visited to ensure residents’ rights are being upheld.

I have been trained to receive complaints and assist residents with resolving problems in situations involving quality of care such as, right to control finances, privacy and visitor rights, transfer and discharge, abuse and other aspects of resident rights.

I will advocate for residents’ rights, resolve concerns about their quality of life and care received, provide education on how to self-advocate and provide education about long term care facilities as well as other service options in the community. I am also able to work with residents, their family or friends and facility staff to meet their needs.

I am here for you – residents, family members, friends, and anyone interested in the welfare of long-term care residents.

My name is Shannon Graf, and I am the Transition Coordinator here at SIILC.
I can provide additional assistance as people transition from institutional care to community-based living.
Some of the services provided can include:

- Attend discharge meetings to assess any needs and work with case managers to ensure a smooth transition.

- Facilitate the transition of individuals with significant disabilities from nursing homes or other institutions to home or community-based residences.

- Assist with needed supports at home, such as home modification and/or assistive technologies.

- Information and referrals to needed services

- Continued support after discharge to help with any future issues.

I am also available to help with Student Transition services. If you need help understanding your IEP, Special Education Law, or information on CASE conferences, I can help you navigate through the special education process.

If your after-high-school plans include job/career choices or further education, I can connect you with resources to help in those areas.
