Throughout this month, we focused on how hospice care supports patients with advanced heart disease by stabilizing symptoms and helping them remain comfortable at home. ⭐️

Providers can refer when symptoms persist despite treatment. Families can ask about hospice before another urgent call or ER visit. Early steps bring more control and greater comfort during a challenging stage of illness.

For families caring for someone with advanced heart disease, changes can happen quickly and feel overwhelming. Shortness of breath, swelling, fatigue, and repeat hospital visits are common signs that more support may be needed. 💜

Hospice can begin before the next emergency. Families can ask about hospice before another urgent call or ER visit and providers can refer patients when symptoms persist despite treatment. ⭐️

You never have to wait for another frightening event to ask about hospice. Support is available long before a crisis.
