Seasons of Life Palliative Care provides supportive care and symptom management of individuals who are dealing with advanced serious illness. While hospice is a needed blessing during an individual's journey, Seasons of Life Palliative Care is NOT hospice care. The program is managed by a certified palliative care administrator with a physician medical director's oversight. Seasons of Life uses an interdisciplinary team to provide supportive care to better support an individual with their physical, emotional, and spiritual needs. Individuals on palliative care can continue with aggressive treatment of an illness including chemotherapy, radiation, blood transfusions, imaging, testing, rehabilitation, physical therapy, total parenteral nutrition, etc., without interruption or having to discontinue those treatments. Seasons of Life Palliative Care works in conjunction with the individual's current healthcare provider(s), specialists, and home health agencies to provide more comprehensive relief of symptoms. Seasons of Life offers both community based palliative care at the individual's residence, as well as inpatient palliative care in a hospital setting. Our team visits patients based on the individuals acuity to insure they receive optimal relief from symptoms such as pain, nausea, vomiting, anxiety, shortness of breath, anemia, anxiety, restlessness, agitation, edema, or any other symptom that accompanies advanced serious illness. The Seasons of Life Palliative Care team will coordinate and make referrals to other community agencies and resources as needed to improve an individual's quality of life.
