Case of the week.
72 year old female who lost her right lateral incisor and failed an implant in another doctor's office. The width of the maxilla was only 1mm requiring bone reconstruction. An implant was placed and grafted with a new graft material composed of human donor bone in a slurry of lineage-committed bone cells creating scaffolding and stimulation to form new bone. The graft was then covered in a bioresorbable membrane barrier. This is next level grafting!

Case of the week...
77 year old female underwent uneventful excision of a left soft palate lesion that was sent for pathology. The soft tissue was cauterized with silver nitrate to stop any bleeding. The next day her husband sent these photos of his wife with normal healing in the soft palate and bilateral upper and lower tire balloon lips. What is the diagnosis and treatment?

Case of the week:
It's increasingly important to scrutinize your doctors training and complication rate for specific procedures. There is more and more mediocrity in the community with bigger groups and more locations and large corporations owning practices. I'm seeing, fixing and serving as an expert witness in more complications than ever! Words to the wise!

Case of the week is actually a letter of thanks from one of several summer interns. I have a great track record getting interns into medical, dental and law school! Carrying it forward one person at a time.

Summer is the best time to remove wisdom teeth because high school age kids have time to heal. Removal of wisdom teeth BEFORE roots are fully grown results in the lowest chance of complications and nerve injuries. Case of the Week:
Minor salivary glands line the oral mucosa by the hundreds. Sometimes, often resulting from trauma, the duct that empties the gland's saliva into the mouth is disrupted. The gland continues to make saliva but it empties into the surrounding tissue causing a balloon effect. The diagosis is a mucocele. The only definitive treatment is removal of the gland.

This is a torus palatinus, a large mass of benign bone that grows in the roof of the mouth. It can grow so big that it outgrows the blood supply from the now very thin tissue on the roof of the mouth. The tissue dies, the bone underneath dies and the only treatment is surgical removal. The patient experienced necrosis on the posterior of the lesion that appears bluish.

The Chief Justice of a Federal Court asked me to reconstruct a parent. It took 6 hours. I removed all upper and lower jaw teeth, placed 10 implants and attached teeth to the implants. The patient had Advil level pain for 1 day. The patient told me 1 week later that she was so happy to smile and feel confident again. Her new friends in assisted living shared they also loved their implants and Dr. Bonacci was their surgeon also! The freedom that a new healthy mouth gives our older population is so rewarding!!

This patient travels to and from Hilton Head Island, SC to see me for her total oral reconstruction. We started in the maxilla, angled implants free hand to avoid the sinuses and placed immediate teeth. 5 hours in the chair. 3 months later I finished the mandible, full arch extractions, immediate implants and teeth in 4 hours. The patients confidence and happiness with her new mouth and soft tissue support of her face is palpable. Seek Board Certified Surgeons for ANY surgery in the mouth for the best outcomes.
