Very rare case of a cemental tear.

#9 was endodontically treated after noting a lateral radiolucency and buccal sinus tract. The sinus tract did not heal after RCT. I raised an exploratory flap, expecting to see the foramen of a lateral canal or a root fracture. Instead I was surprised to see a cemental tear. The loose cementum was removed and I smoothed out the rough root surface.

Patient returned today, asymptomatic after 1 month of healing. No swelling. Sinus tract has healed. Looking forward to the 6 month follow up. Hope we can save this one!

12month recall on a #18, which originally presented as an NP/CAA. Sinus tract was present in the buccal. RCT was completed in Feb 2023. Very happy with the results here, especially when starting with a J-shaped lesion 😃

Apicoectomy of the palatal roots of #'s 2 and 3 completed yesterday.

Patient originally presented 2 months ago with a chronic apical abscess associated with #'s 2 and 3. #3 was previously treated. Draining sinus tract on the palatal. I completed #2 RCT and #3 retreat completed in late January. Multiple rounds of Ca(OH)2 needed for the P canal of #3 prior to obturation due to persistent drainage.

Palatal swelling and sinus tract returned earlier this week 😑🤦🏻‍♂️. Patient was very motivated to save his teeth so we decided on apicoectomy of #3 palatal root. Locating the surgical site was easy due to the large PARL and cortical plate fenestration. The original plan was to only cut #3 but I was able to reach #2 as well. BC putty retrofills. Looking to the future with cautious optimism 😁🤙

Completed endodontic treatment of #7 in August. Follow up today.

18 year old female experienced dental trauma when she was 8, leading to pulpal necrosis and arrested root development. She had intermittent localized swelling events in the B vestibule over the course of a few months prior to initial treatment. Huge periapical lesion with fenestrations in the B and L cortical plates. Dimensions of the radiolucency were approx 14mm X 12mm X 17mm.

I medicated with Ca(OH)2 and placed an apical MTA plug. The soft tissue in the periapical region was firm enough to allow for this type of fill. After the MTA was allowed to set I filled the middle and coronal thirds of the canal with composite.

She has been asymptomatic since the completion of treatment and today's PA indicates significant periapical bone healing. So happy about our progress and looking forward to the next follow up in 6 months!

Some nice results on an apical surgery for #'s 3 (MB root). 4, and 5 (B root) completed in May of this year. Patient returned for a follow up last week. Clinically asymptomatic and apical tissues are healing well 😃

Recall today. RCT #30 completed on 7/26/23. In addition to the two PARLs, a lateral canal exiting the distal surface of the mesial root caused a radiolucency to develop in the furcation area. Great to see things are healing well. Looking forward to another follow up in 6 months.
