*Please Note, this CLS position is employed directly by the consumer to provide services and is not an employee of GCCMH.
This is a Self-directed arrangement where a parent/guardian hires the individual’s workers to provide services and manages the services with the support of a case manager and a Fiscal Intermediary.

Case Manager/Wrap-Around Facilitator
A full-time (1.0 fte) opportunity is available for a Case Manager/Wrap-Around Facilitator in the Outpatient Department. This position is responsible for managing an assigned caseload of consumers aged 0-20 in the children’s department. This position will be responsible for assessing consumer needs, developing, implementing and reviewing individual plans of services, and working with other providers and community resources in meeting/achieving consumer service needs.

The successful candidate must be 18 years of age, possess a Bachelor’s Degree in Social Work and a license as such in the state of Michigan, or ability to obtain a license as a Licensed Bachelor’s Social Worker or Limited Licensed Bachelor’s Social Worker. A graduate from an accredited school of nursing who is licensed and a Registered Nurse in the State of Michigan is also qualified to apply for this position. Two years of relevant experience in a mental health agency preferred and an interest in serving the infant/early childhood population. Must possess a valid Michigan or Wisconsin Driver’s license. Gogebic Community Mental Health offers an extremely competitive benefit package which includes a defined benefit plan.

Direct Care Staff - Overnight Shift (10pm-6am)

Community Mental Health has full-time opportunities in our residential program. Candidates will provide direct care to individuals with intellectual/developmental disabilities and/or mental illness. No experience necessary as we train all of our staff. Starting wage is $20.16 per hour (.50/hr differential) with defined increases, defined work schedule and a tremendous full-time benefit package to include:
