Last week was an interesting one. I got a phone from a female patient asking for health maintenance services. She also told me that she is 36 Weeks pregnant and asked me if I have done a Webster Technique. I told her I haven't done it for a while. I asked, do u have a breech baby? She said yes. So she came in.

She stated that she was 36 weeks pregnant. Babies usually flip head down at about 30 weeks. She also stated that she had tried few therapies to help the baby turn but no luck. So I told her I can not guarantee but I will do my very best. I always try under promise and over deliver. I examined her pelvic griddle, her hip joints along with her round ligaments. I did a very simple pelvic adjustment, work on the round ligaments and some stretching.

Her treatment appointment was Friday with me. Her next ultrasound was the following Monday. She was scheduled to see me on Tuesday. On Tuesday morning, her doctor called her and informed her that her baby flip downward from her latest ultrasound. Her and her husband were extremely happy and avoided a C-Section.

I am really happy for her family and the result could have been coincidental or it was the balancing of the pelvic and release the round ligament tension. Either way, it was great seeing happy patient.

Patient: 43 y.o. Male

Complaint: Patient complaint of mid back pain without any radiation or numbness of the shoulders or the hands. Patient had the pain so severe that he was admitted to the ER two weeks ago. Patient also complaint of weakness and lack of energy with SOB (Shortness of Breath)

Imaging and Test: Patient had x-ray at Kaiser but the films were negative.

Exam: Multiple muscle spasm are T3-18 with limited range of motions. Muscles are tender upon palpation.

Discussion: We can all related to this picture below. I did a focus exam on this patient and pretty much all of the orthopedic tests were negative. However, as mentioned above. Multiple level of muscle tightness and spasmic.

Treatment:
* Soft Tissue Massage
* Stretching
* Manipulation
* Electric Stimulation
* Heat

Home Exercise:
* Rhomboid Muscle strengthening.

Result: Right after the manipulation, patient stated that he can breath better and able to turn his neck in much great range of motion.

Diagnose: Chronic Postural Strain

Note: This is a very common problem with our white collar jobs (Desk Job). Too much sitting in a bad position for too long of a time.

Advice: Take a 15-20 second break every hour and strengthen the rhomboid muscles.

From now on, I would like to write some of my special cases I see and helped in my clinic. I have so many but let me start off with todays special:

Patient: 55 y.o. Male

Complaint: Muscle weakness and tingling feeling of left feet and knee. Having difficulty walking and affecting his daily life.

History: About 8 months ago, patient experienced an severe episode of sharp shooting pain from his anterior left thigh to his left toe. After few months it gotten better but his left leg was still numb and weak.

Imaging and Test: A battery of tests were performed and all of the findings were negative. MRI showed the usual 55 years old spine. NCV showed slowing of peroneal nerve. Patient was seeing a neurologist along with orthopedic doctor but still no solution and the disability continues for 8 months.

Exam: Lumbar examination was negative in all motions along with SLR, Yeoman's Test and few others.

Palpation: Slight tenderness of lateral fibula head with reproduction of numbness.

Discussion: After the history and examination. I have concluded that this patient had two conditions occurred one after another. The initial back pain had created a change in gait and patient over utilized the left foot. Hypertonicity of the perineal muscle created an entrapment. Therefore all of the muscles associated with the nerve were affected.

Treatment:
* Soft Tissue Massage
* Stretching
* Ultra Sound
* Electric Stimulation.

Result: The patient was so happy that his mysterious condition is being solved. He had many sleepless nights and worries due to this condition. He will need about 6 visits with lots of stretching at home.

Patient walked out of my treatment room and stated that he felt his foot was much stronger and stable. Most importantly, a peace of mind knowing what he has is treatable.

Diagnose: Peroneal Nerve Entrapment.
