We're celebrating 50 years!! CHP was founded on Dec. 19, 1974 and the first in-home nursing visit was made in March 1975. In 1984, we were the first hospice established in this area and only the seventh in the state at that time. We'll be sharing more of our history as we celebrate this milestone throughout 2025! ❤🥇

Removing the stigma around hospice is a primary goal of National Hospice Awareness Month. Hospice is not about dying. Instead, it is focused on maximizing the time that a patient has left. That can be months, weeks, or days. The earlier a patient enters hospice, the more likely that goal can be achieved.

There is still enormous stigma attached to hospice. The word is equated with death, and there is fear. But there is so much more to it. Hospice is a holistic approach that's also about the physical, emotional, and spiritual well being of the patient.

As part of National Care at Home Month, we recognize our hospice team members who dedicate themselves to this compassionate work. Hospice care prioritizes comfort and quality of life by reducing pain and addressing symptoms. All medications, services, and necessary equipment are provided, plus bereavement care (grief support) for up to 13 months.

Community Health Professionals (CHP) Home Care & Hospice Tri-County is a non-profit home health & hospice organization serving Auglaize, Logan and Shelby counties.

What makes CHP unique is that we provide a complete range of services - including visiting nurses, in-home therapy, hospice, inpatient hospice, private duty home care and adult day services.

Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
