"Transitions" is the Fauquier County Public Schools post grad special education program, serving students ages 18-22 with intellectual and developmental disabilities. Transitions is a program for special education students ages 18-22 in Fauquier County Public Schools, providing an opportunity for hands-on work experience and life skills education in a community setting. The program will employ res
earch-based methods to develop a Person Centered Plan for each student and work with them on developing skills leading to greater autonomy. Employment
Students will develop pre-vocational and vocational skills in a real-world setting through on-the-job training and work experience.
2. life Skills
We work to build independent living skills through classroom instruction and practice. Skills include:
-Economics and money management
-Caring for Personal needs
-Health and Fitness
-Meal planning and preparation
-Household management
3. Social Skill and Community involvement
During the program, students will participate in community events and recreational activities. Students will be encouraged to explore options and foster areas of interest. Resources may include a local gym, volunteer organizations, libraries, parks and recreation, churches, or interest groups. This community involvement will help provide the student with a support system and an opportunity to build social relationships. Social skills will be taught, reinforced, and practiced in natural settings as the student moves through life in the program.
4. Self-Determination
Self-determination is the belief that one has control over one’s own life. The student will be charged with the task of “becoming yourself.” Students will develop skills required to pursue their goals. Areas include:
-access support agencies
-communication skills
-self-awareness and social awareness
-personal responsibility
-time management
-problem solving
-Self advocacy
