Warsaw Meadows Skilled nursing facility offering advanced dementia care, short term rehab and general nursing care services. From the North, take SR15 south through town.

Warsaw Meadows is located south of US 30 just off of SR 15. Do not follow the turn signs for SR 15. Continue straight past the historic Warsaw Cut Glass Company and follow the road around the corner. You will see Warsaw Meadows on the right. If arriving from the South, you will turn right onto Prairie street from SR 15 just before Marsh Supermarket. Warsaw Meadows will be located on the left.

Congratulations to our therapy graduate, Cliff "Evans" Beavers. Cliff admitted to Warsaw Meadows on March 11. Working hard with our Occupational and Physical Therapists, Cliff was able to complete skilled interventions and increased his independence and safety in self tasks, while also increasing his overall strength and activity tolerance.

Working together with our skilled therapists, Cliff graduated on 5/11! Congratulations Cliff! Your hard work paid off!

We wrapped up National Nursing Home Week with a W***y Wonka Party complete with Oompa Lumpas and Golden Tickets! A scavanger hunt and a party with chocolate cake and assorted candy while we watch the original Charlie and the Chocolate Factory finished out our celebration! Such a fun week!

Day 3 of National Skilled Nursing Home week is in the books. Today, we picked Western Wednesday as our theme. We played wack a mole and got some target practice in with our nerf guns at an outdoor shooting gallery. We finished our day watching a western movie and enjoying some beans and weenies and cornbread we made during cooking club.
