Key Responsibilities
● Serve as operational lead for the interdisciplinary care team.

3–5+ years of clinical experience.
Comfort practicing autonomously.
Telehealth or remote patient management experience.
Strong communication skills with patients and caregivers.
Experience working with complex populations (elderly, disabled, behavioral health).
