Nearly 9 out of 10 postpartum folks have pain when they first return to s*xual activity after childbirth. According to the British Journal of Medicine, up to 36% of birth parents can experience painful s*x 6 months postpartum. Painful s*x, or “dyspareunia” is defined as the persistent ge***al and/or pelvic pain that occurs before, during, or after penetrative or non-penetrative s*xual activity.
Some causes of postpartum dyspareunia are:
❌ Low estrogen
❌ Scar tissue
❌ Involuntary pelvic floor muscle spasms
Some easy tips to help manage these symptoms:
✅ Topical estrogen (with guidance from a doctor)
✅ Scar tissue massage
✅ Pelvic floor stretches

We are excited to announce that we are hiring! Here is a little info about us and who we are looking for....
BCHW is a small, cash-based, private physical therapy practice in West End DC.
We are looking for a licensed physical therapist with >5 years of experience treating patients of all genders with pelvic floor dysfunction and who is comfortable working with LGBTQIA+ identified folks.
The position is part-time (with an opportunity to transition to full-time as the therapist’s caseload builds).
We are particularly interested in hiring a PT with a strong background in:

❇️ Strength training
❇️ Pelvic floor dysfunction & pain (including pregnant & postpartum populations)

To self check to see if you have a diastasis...
Lie on your back with knees bent. Place 2-3 fingers in the middle of your belly button (finger pads facing you). Do a crunch (lifting your head and shoulders off the ground) and see how many fingers you can get between the left and right ‘six pack muscles’ (i.e the re**us abdominus)
Now, it’s important to note that a narrower separation is not always a true reflection of how ‘💪🏾your core is’. For example you could have 4 finger width separation AND still have a strong core or have what we consider as a functional diastasis. This is because you can generate good strength or tension (through your deep abdominal muscles and the connective tissue). So while you do this crunch test and note the finger width separation, also consider assessing..
➡️ The length: How far up (towards your ribs) and down (towards your p***c bone) do you feel the separation?
➡️ The depth: How deep into your body can you stick your fingers before you feel resistance. Is the depth shallow, moderate or deep?)
➡️ The tension from the tissue underneath (i.e.a firm feeling): That midline connective tissue right in the middle of your belly ( linea alba) - when you try to tighten your abdominal muscles, is it firm or quite loose in the middle?
Once you take your fingers away and stay in the crunch, also check if your belly caves in (“doming”); or does it lift upward (“coning”)? Both occur when the linea alba is thinned our and not as supportive as it could be.
These are all very helpful baseline measurements to take to help track changes once you start your own rehab.
The picture in the caption for example is my DRA about 4 yrs ago. Results of my assessment were..
❇️ The width: 3 finger width separation
❇️The length of my diastasis spanned from just under my ribs → p***c bone (but wider at my belly button)
❇️Depth: very deep!😳
❇️ Amount of tension on linea alba: Only mild tension felt and belly ‘cones’ with crunch.

Let’s start by talking about one of the most common and talked about postpartum issues: Diastasis recti. .
Diastasis Re**us Abdominis or Diastasis Recti (DRA) refers to a gap between the two sides of the re**us abdominus (“six-pack”) muscles. For some postpartum folks, DRA can resolve on it’s own post pregnancy. For others, like me..., more support is needed. .
Who’s more at risk of having it? If you..
❇️ carried multiple babies 👶🏽👶🏽
❇️ carried a large baby ❇️ have a small stature and are fit
❇️ are > 35 yrs old .
The causes of DRA?.. Well.., pregnancy is the most common one. As the baby grows and the uterus expands, the re**us abdominus (RA) muscles separate to accommodate its size. The uterus, intestines and the other organs rely on the linea alba (the thin layer of connective tissue that connects the two RA) to hold them in place. This excess pressure coupled with pregnancy hormones, causes the linea alba to soften and weaken.
After the baby is born, the RA separation is still there. About 8 wks later, if the separation is > 2cm (or about 2 to 2.5 finger width) it’s considered a diastasis recti.
The linea alba and core muscles (transverse abdominus, diaphragm, multifidus, pelvic floor) work to gently resist the pressure placed on them from the internal organs. Basically they help to keep your internal organs put! With DRA, the core muscles have a harder time resisting that pressure.
So while DRA on its own is not painful, if it goes unaddressed for a long time it can lead to..
➡️ Hernias
➡️ Low back and pelvic girdle pain
➡️ Urinary incontinence
➡️ Prolapse
