Today was the final day for Farragut Medical.

Like many small practices, we’ve faced ongoing challenges since the pandemic. The recent closure of USAID—and the subsequent loss of many of its subcontractors—has unfortunately been the final blow.

Nicely written article discussing the breast arterial calcifications being offered to all mammogram patients at Washington Radiology. This test is not yet ready for prime time and does not give the information that patients are led to believe.

Please be sure to talk to your internist or Gyn to see if this test is something you should pay an additional $119 to do. Almost every internist, Gyn and cardiologist that I know is saying “No.”

Visit your Gyn annually, but Pap smears may not need to be done at each of those visits.

Women age 21 to 29 should have a Pap test alone every 3 years.
Women age 30 to 65 have three options for testing:
-Pap test and an HPV test every 5 years
-Pap test alone every 3 years.
-HPV testing alone every 5 years.
There are some exceptions to these guidelines, so be sure to discuss your history with your physician to learn what's right for you.

I see my gyn every year because we talk about any symptoms I'm having, especially now that I am in the perimenopause range. She also does a breast and pelvic exam every year. She orders my mammogram and we talk about ways to keep my risk of cancer low (healthy weight, diet of mostly plant-based proteins) and bone health high (resistance exercise, Vit D and Calcium).

The best way to screen for breast cancer is using mammograms. My gyn can also help me calculate my breast cancer risk and decide if I need any extra testing like an ultrasound, MRI or more frequent screening. There is not a good screening test for ovarian cancer which is why I think it's still important to do the pelvic exams even if you don't need to the pap smear. That way he/she can feel your ovaries and notice if there is anything abnormal. To be fair, sometimes it's not easy to feel the ovaries. This is especially true if I've put on weight.

Also, if you feel like your menopause symptoms are not adequately being addressed at your annual visit, know that it's because there may not be enough time to do all of the above AND discuss your menopause sleep disruption, low libido, dry va**na and hot flashes. Please schedule a special visit just to focus on MENOPAUSE. This pro tip has been very useful for so many of my patients who thought they might need to see a menopause expert. Your gyn is your menopause expert!

Allergies or pollen getting to you?

This is the season where a lot of people can come down with symptoms that are often difficult to distinguish from a cold. Have you noticed more sneezing, runny nose, and congestion as well as itching of the nose and palate? You may also experience postnasal drip, cough, irritability, and fatigue. Some patients also have itchy, watery eyes and/or wheezing or a cough from asthma.

There are some simple steps that you might take at home in order to relieve some of these symptoms:

1. A minimally sedating oral antihistamine – examples are Allegra, Zyrtec or Claritin. We recommend the 24 hr version, when possible. Add Sudafed 12 or 24 Hr for additional congestion relief.

2. Glucocorticoid nasal spray – Administered regularly. Examples are Nasonex 24HR (mometasone) or Flonase (fluticasone furoate).

3. Ophthalmic antihistamine drops, such as epinastine, azelastine, emedastine, or olopatadine for the itchy, watery eyes.

4. Regular showers before bed and a sinus rinse (Neti pot or Sinus Rinse) twice daily to clean the pollen out of the nose and sinuses or skin plus keeping your windows closed at home and in the car is also very helpful.
