Individuals in rural areas can face challenges accessing follow-up care or support from community organizations after leaving the hospital, though these services are critical.

A PCORI-funded patient-centered CER study compared standard hospital discharge support — a combination of care instructions to follow at home and help arranging transportation — with a new program that helped people plan their post-hospital discharge needs and manage doctors’ visits.

A PCORI-funded randomized controlled trial compared the return of RA symptoms among adults in remission who stopped taking a type of medicine called a tumor necrosis factor inhibitor, or TNFi, with those who continued them.
