Among Americans hospitalized for worsening heart failure symptoms, 25% are readmitted within 30 days.

A PCORI-funded randomized controlled trial currently underway is comparing two programs aimed at helping older adults with heart failure receive effective follow-up care to improve health outcomes and reduce hospital readmissions:

1️⃣ Mobile integrated health includes a follow-up call from a nurse, access to in-home care by paramedics and virtual medication management by an emergency room doctor.
2️⃣ Care transitions coordinator provides a follow-up call to connect participants with social work, care coordination and home care services.

A PCORI-funded patient-centered CER study compared outcomes for 2 common treatments for aortic valve disease:

◻️ Transcatheter aortic valve replacement (TAVR), which requires no major surgery.
◻️ Surgical aortic valve replacement (SAVR), which typically involves open-heart surgery.
