MAHA ELEVATE, a chronic disease program, will test evidence based interventions focused on nutrition, physical activity, sleep, stress management, substance avoidance, and social connection. Launching this fall and running for three years, the program offers providers a chance to play a direct role in shaping innovative approaches to improving resident health outcomes.

Interested organizations must submit a letter of intent by April 10 and complete a full application by May 15, with the first cohort expected to begin in October.
