What does GUIDE stand for? 𝐆𝐮𝐢𝐝𝐢𝐧𝐠 𝐚𝐧 𝐈𝐦𝐩𝐫𝐨𝐯𝐞𝐝 𝐃𝐞𝐦𝐞𝐧𝐭𝐢𝐚 𝐄𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞. A Program designed to improve the quality of life for people living with dementia and empower families with support and resources.

To participate in the GUIDE Model, 𝐂𝐌𝐒 𝐬𝐭𝐚𝐭𝐞𝐬 that the person must:
1. Have a diagnosis of dementia.
2. Be enrolled in Medicare Parts A and B (𝐧𝐨𝐭 𝐞𝐧𝐫𝐨𝐥𝐥𝐞𝐝 in a
Medicare Advantage)
3. Have not elected the hospice benefit
4. Not be a long-term nursing home resident.
