Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

Our residents enjoyed delicious cookies generously donated by Crumbl and Panera Bread (Washington, PA) 🩷🤍 — the perfect pairing with refreshing juice provided by OSPTA Hospice. 🥤✨
