Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

👑✨ A Prom to Remember at Transitions Healthcare Washington PA! ✨👑

Thanks to the generous support of Heartland Hospice, we rolled out the red carpet for our residents, staff, and their loved ones — and what a night it was! 💃🕺🎉

From dancing to laughter to unforgettable moments, everyone had a fantastic time. One of the highlights? The crowning of two beautiful Prom Queens! 👸👸 We chose them because it was the first prom ever for both — making the night even more special. 💖

Events like this are so much more than just fun — they’re a celebration of quality of life, connection, and inclusion. 💫 Watching our residents light up with joy, feel special, and truly celebrated was a reminder of why we do what we do. 💜
