We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

January at Transitions Healthcare Washington PA is filled with moments to celebrate, connect, and enjoy each day together! 💙

From creative crafts 🎨 and lively games ♟️ to social gatherings, music 🎶, and meaningful activities, our residents have so many opportunities to start the new year with joy and purpose.

We’re looking forward to welcoming back Pathway to Christ Church, engaging with Alicia from OSPTA Home Care & Hospice, and hosting a month full of smiles, laughter, and shared experiences as we celebrate winter and the season ahead! ☃️🤍
