Transitions Healthcare Washington(formerly known as Humbert Lane) is a Skilled Nursing and Rehabilitation Facility that has served the community for over 29 years. We offer short term rehabilitation, long term placement, and personal care. We offer Physical, occupational, speech, and respiratory therapy. Our mission is to provide high quality and innovative care to our residents enabling them to transition to recovery in a comfortable customer service oriented environment.

Today, we proudly celebrated Veterans Day with a special pinning ceremony in collaboration with our friends at Amedisys Hospice 💙❤️. Each of our resident veterans received a certificate recognizing their branch of service and an American flag pin, presented with gratitude and respect for their courage and sacrifice. 🎖️🇺🇸

🎃 Pumpkin fun at Transitions Healthcare Washington PA! 🎃

This morning, our residents teamed up with our amazing activities assistants — Paula and Kurt — to create their very own pumpkin designs! 👩‍🎨🧑‍🎨They drew out a spooky or silly face ✏️, then rolled up their sleeves to scoop out the “pumpkin gunk” and pick out the seeds! 😄🪣

The finished masterpieces are now proudly displayed in our dining room 🧡🍂 — and staff will be voting on their favorite throughout the day! 🗳️✨

We love seeing so much creativity, laughter, and teamwork filling our halls this Halloween season! 👻🎃
