🌍 August is — a time to honor the generosity, leadership, and impact of Black changemakers in our communities. Today, we're highlighting Montez Carter of Trinity Health Of New England and board member of United Way of Central and Northeastern Connecticut.

Over 20 years ago, Montez volunteered at an interfaith ministry community kitchen. That work introduced him to where he saw how collaboration could amplify . “That word ‘United’ in United Way is the most powerful word. When we bring our different talents and perspectives together, we can make a real difference.”

Montez has stayed engaged with United Way ever since, believing deeply in the responsibility to give back — not just financially, but through time, expertise, and leadership. He reminds us that the opportunities we enjoy today exist because of those who sacrificed, gave, and persevered before us.

With a focus on improving care for patients with opioid and stimulant use disorders, Trinity Health Of New England was proud to contribute to the development of a national toolkit—created in partnership with three clinical teams and supported by the Centers for Disease Control and Prevention CDC.

Trinity Health Of New England was chosen for our implementation of Peer Support Specialists (PSS) who are integrated into the care team and work to interrupt the cycle of readmission. Upon discharge from treatment, patients are connected with rehabilitation centers, outpatient programs and clinics. PSS ensure that patients have appointments scheduled and ensure continuity of care to prevent lapses in treatment. As part of the care team, PSS assist in providing wraparound support for barriers like transportation, housing instability, and insurance issues, ensuring patients remain engaged in their recovery.

“Internal, interdisciplinary collaboration was key to our success especially as we built a model that leverages Community Health and Well-Being’s approach to integrated peer-to-peer support within the inpatient care team,” said Carolyn Alessi, MBA, Regional Director, Community Health and Well-Being & Curtis D. Robinson Center for Health Equity. “Each member of the team brought thoughtfulness and insight to identifying gaps, addressing systems barriers, and surfacing meaningful opportunities for improvement.”
