My name is Jeanette Sergent and I’m the Administrator/Coordinator at Riverwalk Senior Living in Waterloo, we currently have 29 amazing residents!

We’re currently planning out to December and I’m looking for a few things to help make the residents feel festive and loved!

Donations of any craft things or games/ things you know your grandma and/or grandpa would LOVE!!

Christmas cards/ small presents sent to residents

Volunteers (adults, teens, kiddos! Everyone is welcome with a mask on, to come in and chat/ do an activity with us or even plan your own activity and share it with us!

Guest speakers and live musicians as well!

Position: Lead Resident Assistant / First and Second Shifts / Full-time

PURPOSE OF YOUR DESCRIPTION

To initiate and assist Resident Assistants in their job duties and monitor changes in the well-being of our residents and report to the Administrator issues or concerns that may affect the daily operations of Riverwalk Senior Living.

SPECIFIC REQUIREMENTS

- Must be able to read, write, speak and understand the English language.

- Must possess good customer service skills, honesty, the ability to deal tactfully with personnel, Residents, families, visitors, government agencies/personal and the general public.

- Must not pose a threat to the health or safety of other individuals in the workplace.

- Demonstrates basic knowledge of documentation forms.

- Possess the ability to share knowledge and teach others.

- Demonstrates accountability for own actions and is receptive to feedback.

- Must demonstrate basic computer skills and knowledge.

- Must possess leadership and organizational skills.

GENERAL RESPONSIBILITIES

- Greet all visitors with respect.

- Must possess the ability to organize the programs, goals, policies and procedures that are necessary to maintain an informed community.

- Must possess leadership ability and willingness to work harmoniously with other personnel.

- Monitor Resident condition and advise care team of such.

- Report any concerns related to staff or Residents to care management team.

- Execute a good role model for all staff including completion of MAR, progress notes, and full and complete shift report.

- Attend and participate in all staff meetings, training, and/or in-services.

- Monitor medication cart weekly for items laid out by care team management.

- Monitor MAR daily for appropriate transcription of orders and discontinuation, staff initials of scheduled and PRN medication documentation.

- Destruction of expired or discontinued medications and record such on appropriate forms.

WORKING CONDITIONS

- Communicate with staff to support management and company policies.

- Is subject to remain in place or call-back during emergency conditions (severe weather, evacuation, fire, sudden illness, and accident).

- Attends and participates in mandatory meetings, in-services, and all trainings.

- Is subject to injury, falls, burns, odors, etc., throughout the workday as well as a reaction from dust, disinfectants, and other air contaminants.

- Is subject to exposure to infectious waste (body and bodily fluids), diseases, conditions, etc., including but not limited to TB, AIDS, and the Hepatitis B viruses.

- May be subject to the handling of and exposure to hazardous waste materials.

- Maintain a liaison with the residents. Families, partnerships with outside agencies, etc., to adequately plan for the resident’s needs.

Title: Resident Assistant / All Shifts / Full-time and Part-time

Background:

The Resident Assistant (RA) is an integral part of the service delivery of Riverwalk Senior Living. The RA’s primary function is to inspire independence in the provision of cares’ we provide to our clients. All services must be provided in a manner that reflects the professionalism, philosophy, and mission of Riverwalk Senior Living.

Job Duties

Core:

-Comply with all departmental and organizational policies and procedures.

-Achieve and maintain compliance with all local, state, and federal regulatory and other requirements.

-Observe and report resident changes of condition to the R.N. Document changes and reporting of such.

-Provide home care services to frail elderly and other clients as assigned by the R.N. Document care provision.

-Attend all staff meetings/training sessions or complete make-up work.

-Obtain vital signs and document and report as assigned by the R.N.

-Assist home care clients with activities of daily living and instrumental activities of daily living as assigned by the R.N.

-Administer medications to stable home care clients and document as assigned by the R.N.

-Report and document incidents, errors, and accidents on the Apara Care Health at Home Occurrence Report Form.

-Maintain Certified Nursing Assistant status as required and maintain compliance with 12 ongoing hours of education per annum.

-Maintain client and company confidentiality at all times.

Peripheral:

-Maintain calm, helpful approach with clients and families even in times of extreme stress and unpredictable situations.

-Receive, process, complete, and follow up on multiple requests and tasks as assigned by R.N. or requested by client, client’s family, physician, or other members of the care team.

-Assist the Apara Care Health at Home team with marketing efforts of all departments as assigned.

-Attend at least one external educational job-related offering.

-Report absent or late shifts at least two hours prior to assigned shift start time.

-Represent Apara Care Health at Home and other departments in a professional, positive manner at all times and exemplify teamwork.

-Obtain and report alternate coverage for uncovered shifts as assigned by the R.N. or designee.

-Any other task as assigned by R.N. or administrative team.

Job Requirements:

Education, Experience, and Accountability:

At least twelve months experience working in a skilled nursing facility, assisted living facility, or rehab center preferred but not required. The RA reports directly to the Care Coordinator/RN.

Mandatory Physical and Ancillary Requirements:

-Complete pre-placement physical exam and obtain negative tuberculosis skin test or chest x-ray.

-See, smell, and hear at a level sufficient to complete the core and peripheral job duties. This includes the ability to give and receive verbal and written instructions, hear audible emergency signals, alarms, telephone, and other sounds.

-Lift, transfer, carry, and position clients up to 250 pounds. Push/pull clients weighing up to 300 pounds.

-Use home care equipment as assigned including hoyer lifts, electronic lifts and stands, scales, wheelchairs, and vital sign monitoring equipment.

-Stand and walk 80% of an eight-hour shift.

Position: Kitchen Lead

PURPOSE OF YOUR JOB

The primary purpose of this position is to perform work of moderate difficulty in regards to the preparation of foods on a large scale, commercial- type, under general supervision. Work is performed according to specific policies and procedures, producing food according to menus and standardized recipes. Responsibilities include preparing ingredients, cleaning the cooking area, meal planning and serving food. Work is done under the supervision of the Community Manager.

SPECIFIC REQUIREMENTS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

Knowledge of meal planning, food preparation and cooking methods on a large-scale basis.
Knowledge of the materials and equipment used in large scale cooking operations.
Knowledge of health and safety standards utilized in a food service operation and in the facility.

Prepare menus in a rotation including all recipes necessary to carry out menu cycle.
Prepare the grocery list according to the menu.
Required to grocery shop as needed.
Prepare food orders and submit to vendors chosen by management.
Follows and stays within a monthly food budget.
Monitors and records freezer and refrigerator temperatures as required by the state and report any discrepancies to management.
Prepares appropriate foods, hot and/or cold, in a timely fashion for meal service.
Is responsible for cleaning tables and equipment; every day maintaining a sanitary condition in the work area.
Operates various kitchen equipment and reports malfunctioning equipment to management.
Ability to weigh and measure ingredients properly using recipes in order to produce a standard product.
Interviews residents and staff for meal acceptance and attends Resident Council & staff meetings monthly.

Job Description
Position: Resident Care Coordinator

PURPOSE OF YOUR JOB

This is a full time position, at hours determined upon need. The Resident Care Coordinator is responsible to work with the Administrator in developing, establishing, and maintaining excellent service for the Resident’s of Riverwalk Senior Living and compliance with the DHS 89. The Resident Care Coordinator reports directly to the Administrator.

SPECIFIC REQUIREMENTS

Must be able to read, write, speak, and understand the English language.
Must possess good customer skills, the ability to deal tactfully with personnel, resident’s family members, visitors, government agencies/personnel, and the general public.
Must possess the supervisory ability and the willingness to work harmoniously with and supervise personnel.
Must possess the ability to implement and interpret the programs, goals, objectives, policies, and procedures, etc. that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to address challenging situations and report to the Administrator.

Set up new Tenants in ECP- demographics, care plan, daily tasks, e-mar, etc
Manage all aspects of Tenant charts- both paper and electronic.
Keep compliant with policies and procedures.
Organize and participate in Tenant care team meetings.
Assist in the assessment process and develop Care Plans to meet Tenants' needs.
Educate staff on care plan tasks, oversee compliance of tasks, and review documentation daily.
Oversee all communication with Tenants' P*P.
Oversee all aspects of incident reporting.
Oversee the compliance of emergency planning.
Provide direct care services to Tenants' as needed.
Provide hands on training for continuing education.
Assist with developing daily activities and direct implementation of activities.
Participate in the orientation of new Tenants’/family members to the facility.
Oversee transportation needs..
Assist in the orientation of new employees.
Monitor all aspects of medication administration/management.
Assist with training and medication shadowing of staff who have completed proper medication training.
