My Choice Wisconsin Family Care and Partnership members, your voice matters! Help us make sure you have the best health care. You can do this by taking the 2025 Department of Health Services (DHS) Member Satisfaction Survey.

Watch your mail. You may get a letter from DHS asking you to fill out and return the survey. You can return survey response comments by mail to DHS in the envelope included. You can also scan the QR code and follow instructions to fill out the survey online.
