Happy Mother’s Day to the women who quite literally hold the world together.

As a spine surgeon, Dr. Samuel spends his days thinking about alignment, strength, and support systems. But nothing compares to the quiet engineering of a mother’s care: constant, resilient, and foundational.

Most patients don’t “need a neck fusion.”

That’s the misconception. An anterior cervical discectomy and fusion (ACDF) isn’t about “fusing the neck” - it’s about relieving nerve compression.

When a cervical disc herniates or degenerates, it can press on spinal nerves or the spinal cord, causing arm pain, numbness, or weakness. The goal is to remove what’s causing the pressure, not simply “fix the spine.”

In ACDF, the surgeon removes the damaged disc from the front of the neck, eliminates pressure, and rebuilds the space for stability. The fusion is a secondary step that allows the segment to heal in a stable position.

ACDF isn’t a “last resort fusion procedure.” It’s a precise decompression surgery that restores space for the spinal cord and nerves when conservative care fails.

The real question isn’t “Do I need a fusion?”
It’s: “Is there a structural compression that needs to be relieved?”

ACDF Surgeon Do you suffer from a herniated or degenerative disc in the cervical spine, or neck area? If so, you may benefit from a procedure called Anterior Cervical Discectomy and Fusion or ACDF. The main goal of ACDF is to alleviate pressure, reduce symptoms, and improve the patient's quality of....

Patients with chronic back or leg pain due to mechanical instability or nerve compression may be candidates for transforaminal lumbar interbody fusion (TLIF).

Common indications include degenerative disc disease, vertebral slippage (spondylolisthesis), spinal stenosis with instability, recurrent disc herniation, and select fracture or deformity cases.

The key distinction is the failure of conservative care combined with a clear clinical-radiographic correlation.

TLIF Surgeon Transforaminal Lumbar Interbody Fusion (TLIF) is a good option for those experiencing chronic lower back pain that has not responded to conservative treatments like physical therapy or injections. Lumbar synovial cysts are fluid-filled sacs that can develop in the spine and may or may not cause symptoms.

When symptoms do appear, they often include:
· Lower back pain, especially worse when standing upright
· Pain that radiates into the leg (sciatica), usually on one side
· Numbness, tingling, or muscle weakness in the lower limbs
· Reduced ability to stand or walk comfortably

Although uncommon, severe nerve compression can lead to cauda equina syndrome, which requires urgent medical attention.

When Pain Outpaces Imaging

Fusion patient returns with worsening axial pain.

Radiographs underwhelming.
CT reveals pedicle screw loosening and rod stress failure.

Decision: revise for pseudarthrosis.

Outcome: improved stability and symptom relief.

Lesson: In spine surgery, persistent pain should override “normal” X-rays

Failed Spinal Hardware Specialist If you've been diagnosed or believe you have a loose screw or a broken rod following spinal surgery, it's crucial to seek expert medical intervention to address these complications. Thoracic disc herniation explained:
• Occurs in the 12 vertebrae between neck and low back
• Intervertebral discs act as shock absorbers
• Age-related weakening can lead to disc bulging
• The nucleus may protrude toward the spinal cord

It is rare, occurring in less than 1% of all disc herniations.

Thoracic Disc Herniation Specialist A thoracic disc herniation can occur when the spinal disc bulges or ruptures into the spinal cord causing nerve irritation. Most patients benefit from conservative therapies, however, some individuals may require surgical intervention Doctor Andre M. Samuel, ortho...

Cervical radiculopathy is caused by nerve root compression in the neck.

Most common mechanisms:
• Herniated or bulging disc
• Degenerative bone spurs (arthritis)
• Age-related disc height loss and dehydration

Less common:
• Acute trauma or repetitive mechanical strain causing disc injury

Result: reduced space for the nerve root → pain, numbness, or weakness.

Cervical Radiculopathy Specialist Do you have pain or a pinched nerve in your neck that is causing arm pain and weakness? If so, you may have a spine condition called cervical radiculopathy. This condition, if left untreated can cause disability and loss of function. Cervical Radiculopathy specialis...

As a spine surgeon, Dr. Samuel often sees patients struggling with persistent arm weakness, numbness, or neck pain.

Posterior cervical decompression and fusion may be the right solution for those whose symptoms persist despite conservative treatment.

It’s effective for:
• Cervical spinal stenosis
• Disc herniations
• Traumatic injuries
• Spine instability

Careful evaluation is key and surgery is only one piece of the treatment journey.
