2 year anniversary of this crazy leap of faith! In that time I have gained 140+ patients who understand the value of personalized care, diagnosed a melanoma that was a “wart” mentioned at the end of a visit for a cough (she’s fully recovered), met a patient for multilayered sutures for a deep cut so they could avoid the ER, had the time to listen to many with depression and anxiety for an hour+ each instead of admin dictated 5 min visits, and just today (Saturday) met a patient to start IV fluids so she could avoid the ER during flu season.

I no longer need to pick up shifts in the ER or hospital/ICU. Because of your support I am able to be at this wonderful clinic 4 days a week!

Sorry for the ramble but I’m over the moon happy with my career and thankful for my lovely patients! I am still accepting new patients for primary care and weight management.

Several people have asked if they can use their health insurance for our office visits. Unfortunately we would have to employ a small army of people just to fight with insurance companies to get approval for visits- that’s why we use a monthly membership/direct care model instead. This keeps costs low for everyone.

Of course. Insurance companies take your high premiums and refuse to pay for your treatments even after approving the treatment! And now premiums are going thru the roof with the same crappy service.

Blue Cross authorized mastectomies and breast reconstructions for women with cancer but refused to pay the full doctors’ bills.

But the insurer did make some exceptions. On several occasions, Blue Cross executives signed deals to pay for their wives’ treatment.

The White House announced agreements with Eli Lilly and Novo Nordisk that will change how patients access and afford GLP-1 medications.

Four Pathways for Patient Access:

1.TrumpRx.gov (Launching Jan 2026) This will be a government-run, direct-to-consumer cash-pay website.

- Injectables (Wegovy/Zepbound): Starting at ~$350/month, trending down to $245/month over two years.

- Oral GLP-1s (once approved): Starting doses will be priced around $150/month.

2. Medicare Coverage (Starting Mid-2026) For the first time, Medicare will cover these drugs for obesity through a CMS pilot program. This is an amazing milestone that we’ve all been waiting for.

- Patient Cost: A flat $50/month co-pay.

- Program Cost: Medicare will pay a rate of $245/month.

- Who Is Eligible? The pilot will target higher-risk beneficiaries, initially about 10% of the Medicare population. The criteria will be narrower than the FDA label, focusing on patients with:

- BMI >27 with prediabetes or established cardiovascular disease.
- BMI >30 with uncontrolled hypertension, kidney disease, or heart failure.
- BMI >35 (severe obesity).

3. Medicaid Coverage

- States will have the option to get the same low price of $245/month for GLP-1s for all covered uses.

4. LillyDirect (New Prices Available Very Soon)

- As a separate cash-pay option, Lilly has lowered its price for Zepbound to $299/month for the lowest dose (up to $449/month for higher doses). This is an immediate option for those without coverage.

- Zepbound will be available in a new multi-dose pen so perhaps this will replace the vials and make it easier for patients to administer. (Awaiting FDA approval)

Pipeline & Regulatory Updates:

Fast-Tracking Oral GLP-1s: The FDA has granted priority review vouchers to speed up the review process for oral Wegovy and Lilly’s orforglipron, shortening approval timelines.

We are coming up on open enrollment, and patients need to know about things like this. Don’t get United or any other “advantage plan”. Stick with plain Medicare add part D for drug coverage if you need it.

Most importantly- find a physician who is direct pay, it’s almost always more affordable to have a direct primary care doctor than to pay all the copays/deductibles that insurance requires. PLUS you won’t have a nurse practitioner, PA, or doctor who work at the mercy of the insurance company.

According to a report from The Guardian, UnitedHealth has allegedly been pressuring nurse practitioners to flip elderly patients’ status to “Do Not Resuscitate,” even when these patients have explicitly requested to be resuscitated.

I sometimes forget what 50 pounds of weight loss is really like! I took a look at my drivers license picture from two years ago and my new tribal citizenship card from today and I thought about all the work I’ve put in exercising, increasing vegetables(which is very hard for me), cutting out pop completely, reducing alcohol, drinking mainly water, trying my best to get good sleep, and adding semaglutide. Forgetting the lack of makeup and overexposed picture on the right I see such a difference in the amount of chubbiness in my face.
I see someone who no longer has to take cholesterol medicine.
I see someone who is no longer on the verge of needing blood pressure medicine .
I see someone whose joints feel better.
I see someone who can jog and lift weights so much more easily than before.
I see someone who went from 210 pounds to 160.
I see someone who took an insane risk by quitting her old hospitalist job and starting a clinic from nothing.
I see someone who has helped so many other people on their weight-loss journey and I’m proud of all of them and her!
