PreNatal Care Coordination & Child Care Coordination (PNCC & CCC)

What is Prenatal Care Coordination? Prenatal Care Coordination is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby. Whitmore Cares Goals:
* To decrease the incidence of low birth weight infants
*To provide guidance and support to expectant mothers
*To encourage expectant mothers to take an active role in their health and the health of their baby
*To give babies a healthy start in life

Prenatal Care Coordination services include:
• Help getting the health care you need.
• Personal support.
• Information on good eating habits and health practices.
• Information and help finding needed services in your community. The care coordinator will help you hby asking about your:
• Current and previous pregnancy.
• Health history.
• Relationships.
• Worries. If you are eligible, the care coordinator will help you get the services you need. There is no copayment for this service. Why are Medicaid and BadgerCare Plus doing this? Pregnancy is a time of change, every woman can use help with:
• Planning for a healthy pregnancy, birth, and beyond.
• Getting ready for a new baby.
• Lifestyle changes.

*A care coordinator can help you during these times.
*A care coordinator will listen to your concerns and answer your questions. A care coordinator will work with you one on one to provide a personalized, confidential plan of care. Questions that a care coordinator can help you with:
* When should I go to the doctor?
*How will I get to the doctor?
*What are signs of labor?
*How does this work if I am in an HMO?
*What community services are available to me?
