Des Moines County Living Well

Des Moines County Living Well Vision – To be a vibrant community where residents live a life focused on physical activity and healthy eating.

Des Moines County Living Well is focused on addressing concerns that were voiced during the community health needs assessment completed in late 2015. The focus areas include:
• Obesity and Diabetes
• Mental health and substance abuse
• Poverty
• Tobacco use
• Violent Crimes

Des Moines County Living Well received a state-level grant called the State Innovation Model (SIM). As part of the grant, the group is tasked to form a Community Care Coalition (C3) in which we can discuss issues that directly impact the community. Other areas of focus include care coordination, medication safety, and addressing the social determinants of health. Previous initiatives/collaborations include:
• Community Gardens – Hy-Vee, YMCA
• Bike Paths – Bike Burlington, Edward Stone School
• Complete Streets
