Last week Tandigm held our annual Holiday Raffle Luncheon. Thanks to the incredible generosity of our teammates, we raised $9,668.00. These proceeds will support Cradles to Crayons, Philabundance, St. James School, and Blue Sky Surgical— in honor of our founder, Tony Coletta, MD. Props to our Activity and AWARE teams for a great and giving-focused event!

At Tandigm, teamwork drives patient outcomes. When a P*P noted a patient in need during a Tandigm office visit, our teammate looped in the Nurse Care Manager. She quickly connected with the patient to educate, support and add them to our Heart Failure Pathway.
