At Tandigm, teamwork drives patient outcomes. When a P*P noted a patient in need during a Tandigm office visit, our teammate looped in the Nurse Care Manager. She quickly connected with the patient to educate, support and add them to our Heart Failure Pathway. October is - a perfect time to celebrate our hardworking pharmacy team. We’re grateful for our pharmacists and pharmacy techs who play a vital role in medication review for patients after discharge.

Recently, a teammate caught a dosing error, then coordinated with the patient’s pharmacy to ensure they received the correct dose, with the goal of preventing adverse events and ED/hospital visits. Thank you to our Pharmacy team for your focus on patient safety!
