Today is 6/12/2025 - There is currently an issue with our phone service. We are unable to make or receive calls in any of our locations. If you have an emergency, please call 911.

All other routine items please contact your provider through My Chart. New Patient visit requests or Follow up Visit requests please visit our website and submit an appointment form someone will call you as soon as we can.

Hello Please Note that our Avon, Manchester, Hamden and West Hartford offices will be closing at 12:00 pm on Thursday 2/6/2025 due to weather. Our Middletown location will be closed all day.
If you need help rescheduling your appointment please call us at 860-232-9911 starting at 8:00 am on Thursday.

September 28th is National Penicillin Allergy day! If you or a loved one have a history of rash while taking a Penicillin family antibiotic, you might not actually have an allergy. Please schedule an appointment to discuss further with one of our providers.
