This holiday season in particular, comes at the end of a year that wasn’t the best. But it turns out, the holiday traditions feel comforting even though they were a bit harder to pull off this year. For my office, it’s the kidnapping of our office elf (Petunia) by the Reindeer Bandits the day before Christmas Eve. They hold her for ransom ie food that we all bring in for the office potluck. Getting into the ugly Christmas sweater theme warmed my soul and celebrating a month of riddles and laughter with my work family was a blessing. Happy Holidays from the Mariwalla Dermatology family.

Halloween but make it Zodiac. Thanks to Joanie Baloney face painting for bringing our signs/spirit animals to life. Had a bunch of Ta**us representing but got a little sprinkle of Virgo, Pisces,Cancer, Capricorn and some spirit animals like the fox and cat. Lucky to work with this amazing crew.

Compost. I got some not great news today and it feels like a setback. Well, let’s be real - it is a setback. So today I’ve been thinking about compost.

Like how does putting rotting trash and sh*t on a plant make it bloom more? How does rot create the right environment for even better flowers?

I let myself have a moment. For what it’s worth my first shot of Lupron did not help said moment. 🤦🏽‍♀️ But then I decided I have to be the plant. In this process there are hills and valleys. Today has been a valley.

Tomorrow I climb back up the hill. Let’s see what blooms….

In many ways cancer isn’t just about the big C word and trying to stay alive; it’s also about these small papercuts (as I like to call them) that catch you off guard and sting. For me one of the stingers was taking off my wedding band for the first time in 13 years. It’s a stupid little thing that isn’t a big deal …. Like a papercut…. But it gets you every time you look down at your hand. Like not putting back on a locking bracelet that my husband got me when my first child was born in case I need another MRI and have to take it off again. Small. Little. Papercuts.

But today I got the wedding rings back on. And I am only post op Day 4. “Feeling good, Louis” as they say. Thank you for all the kind comments and texts and all the things. It takes a village and I am blessed to have a pretty amazing one.

When you have to cancel patients suddenly and can’t attend a conference you helped organize, word gets around. Figured it was time to tell you myself.

A patient told me that when you are diagnosed with cancer you become the person you need to be at that moment. You don’t know how you will fight it, or if you can, but when the moment comes you will rise.

Well a few weeks ago I became a 47 year old physician with breast cancer. And today I rise to the fight. I got this.

Saw a lovely patient the other day who was a little bit surprised I knew her diagnosis and what skin signs to look for. A recent study showed that women and girls with this often present with an increase in acquired melanocytic nevi, keloids, lymphedema and vitiligo. She was kind enough to let me take this photo. Let there never be a doubt where I stand on DEI in Dermatology. South Asians are a minority population in the United States but we are overrepresented in medicine. So even though I may not be able to apply to programs for underrepresented minorities in medicine (“URIM”) I believe in DEI efforts. Let me break it down:
Diversity enhances medicine. Period. The data is plentiful and backs this up.
Equity. Let me explain the analogy made at our meeting yesterday. There is a fence that is 6 feet tall. And you tell everyone they must get over the fence. But you give everyone the same size step-stool. Well the people who are taller will have an advantage and be able to get over it quickly. Heck, some people won’t even need the step-stool. Those who are shorter will still be stuck. If you believe in equal opportunity you leave the people who couldn’t get over the fence behind. If you believe in equity you give people appropriately sized step stools so everyone gets over the fence. I want Dermatology to be a place where everyone gets over the fence.
Inclusion. I mean, obviously.
Dermatology is better than the fight we are having with each other. Dermatology deserves the public trust. Dermatologists are genuinely interested in advancing patient care. What I see playing out in the public sphere breaks my heart. So I will wear this sticker each day of the conference because the fence should not be a barrier for anyone. And if you need a boost over it because your step-stool isn’t right-sized, I’m your girl.

National Sons Day. Let’s be honest, this trio gets celebrated every day. Over the years, I’ve worked hard to earn a bunch of titles. But the one I’m most thankful for? Mom. Or as my boys have now started calling me: Bruh. When I objected and said I don’t want to be “bruh”, they said okay broh-sina. 🤦🏽‍♀️

Studies show that one of the most successful treatments for melasma is in fact regular and diligent use of sunscreen. I recommend one that is tinted for maximal benefit.

Sometimes we get so focused on what lips look like facing someone that we forget to look at the profile. Enhancing a lip is about fixing normal anatomic variations and bringing a sense of proportion back to a profile. In the top photo my gorgeous patient had an upper lip that appeared set back and smaller than what her face likely needed. In the “after,” we were able to bring back some beautiful lip harmony while not making it look like we were ever there. The ghost effect.
