✨ Heather’s Facelift Journey — Day 5 👀 ✨

Five days out from surgery, Heather is healing
beautifully. The swelling is expected and temporary,
and even through it, her jawline is already starting to
show definition.

Her case was one of the more complex ones I’ve done.
Years ago, she underwent a thread lift (MyEllevate) to
tighten her neck bands. Those permanent sutures
tethered her platysma muscle, creating jowling and
restricting movement. During her facelift, I carefully
released and removed those sutures from both the skin
and the muscle to restore natural structure and allow
for a proper lift.

One day out from surgery, Heather is sore but in good spirits. She knows every bit of this swelling and bruising is temporary — and that what’s ahead will be worth it.

Heather’s case was a particularly complex one. Years ago, she had a thread lift procedure (MyEllevate) to address her neck bands. Unfortunately, the permanent sutures from that procedure ended up making her jowls worse by tethering the platysma muscle in the wrong places. During her facelift, I had to carefully remove those sutures — releasing them from both the skin and the muscle — to restore the foundation and allow for a proper lift.

Thread lifts can sometimes create a temporary “tightened” look, but they don’t correct deeper laxity. In fact, as in Heather’s case, they can anchor the tissues in unnatural ways, causing jowls or distortions that make surgical correction more challenging later. Removing these sutures is delicate work but essential for achieving a smooth, natural contour.

A Difficult Facelift Case - And Why It Was Worth It ✨
Kim was 58 when she came to see me. She’s active and healthy. 5 years ago, her face and neck started changing fast—jowling, loose skin, hollowing. That’s when the spiral began.
She was told non-surgical treatments would be the solution. She tried everything:

💉 HA Fillers
⚡ Morpheus8
⚡ Ultherapy
💥 CO2 laser
💥 Fraxel
💥 IPL

Nothing worked. And worse, they layered damage.

By the time she came to me, Kim had:
– 8 years of Filler fatigue
– Morpheus8 which likely melted precious fat under the skin
– Ultherapy treatments which layered more scarring in the deeper plane where I needed to perform the lift
– Dozens of squamous cell carcinomas removed from the face and body
– Aggressive lasers on fragile, Irish skin

Eyelid Rejuvenation + Brow Lift at 38 👀

This beautiful 38-year-old patient of mine had an Upper Blepharoplasty and Endoscopic Brow Lift. She came in for a consultation to discuss her upper eyelid hooding, lower eyelid skin crepiness, and the asymmetry in her eyelids. She expressed concerns about her upper eyelids feeling heavier, and one side is droopier than the other. She also noticed that her left brow sits lower than the right and asked whether a Brow Lift might be beneficial for improving symmetry.

👀 “My eyelids feel heavy… and I just want to look like me again.” This patient came in feeling like her eyes had aged faster than the rest of her face. She didn’t want to erase all signs of aging — just to look more like herself again.

✅ What I Recommended:
• Endoscopic Brow Lift – to lift her brows back to a natural, more open position and soften the heavy appearance
• Upper Eyelid Blepharoplasty – to remove excess skin
• Lower Blepharoplasty with Fat Transposition – to reposition the fat pads (eye bags) under her eyes and smooth out the tear troughs
• Skin Pinch & Lateral Canthopexy – to remove loose skin under the eyes and support the lower lid

🧠 EDUCATION MOMENT: Many people think droopy eyelids are just a “skin issue,” but I find that brow ptosis is often the root cause. When the brows fall, they push down the upper eyelids and create that tired or hooded look. In this case, even after lifting the brows, she still had excess upper lid skin — so we combined the lift with a conservative blepharoplasty.

For the lower eyelids, her fat pads were creating puffiness, and she had a true tear trough deformity — which we addressed by transposing the fat rather than removing it. This keeps the result smooth, natural, and avoids hollowness.

After years of using filler to mask under-eye hollowing, this 53-year-old patient was ready for a more permanent solution. She wanted to look refreshed—like herself, just more awake.

✅ What She Had Done:
✔️ Upper Blepharoplasty
✔️ Lower Blepharoplasty with Fat Transposition
✔️ Lateral Canthopexy

💬 “People kept saying I looked tired… even when I wasn’t.”

✨ Her Concern:
Filler helped temporarily, but it started creating puffiness under the eyes. The hollowing and the heaviness of her upper lids made her look more tired than she felt. She wanted something long-lasting and natural—not a patchwork fix.

📚 EDUCATION MOMENT:
Filler can’t fix everything. In patients with under-eye bags, volume distortion from old filler can actually make things look worse over time. For her, the first step was dissolving all remaining HA filler to reset the area.

Then we addressed the real issue:
➡️ Lower Blepharoplasty with Fat Transposition smoothed the contour beneath the eyes by repositioning herniated fat.
➡️ Upper Blepharoplasty removed the excess skin weighing down her eyelids.
➡️ Lateral Canthopexy supported and reinforced the shape of the eye for long-term results.

🔥 Eyelid Rejuvenation at 43 - Lower Blepharoplasty 👀

This handsome patient of mine came to me because he was constantly being told he looked tired, despite feeling young. He had tried everything: eye creams, anti-aging serums, even under-eye filler three years ago. Nothing worked.

He wanted a solution that wouldn’t leave him looking “done.” He wanted to look natural. After evaluating his anatomy, I recommended ✅ Lower Blepharoplasty with Fat Transposition where I used the excess fat under the eyes and repositioned it to smooth the undereye hollows and contour.

The result? A refreshed look and a longer lasting solution. No one is asking if he’s tired anymore—his energy finally matches how he looks.

💬 Education Moment:
In this case, avoiding a skin excision was critical due to his facial anatomy. Given that he has a negative eye vector, further skin excision could put him at risk for ectropion, and the possibility of a visible incision line.

If you’ve had filler that didn’t help or made things worse, lower eyelid surgery with fat repositioning and/or facial fat transfer are other possible options.

👀 Should You Combine Facelift and Rhinoplasty? The answer is—it depends!

Larry came in with two big concerns: the loose skin on his neck after weight loss that made him look older than he felt—and a bulbous, prominent nose he felt never quite fit his face. As a competitive rugby player (yes at 60!), Larry and I worked on a plan that would deliver great results without compromising his lifestyle or timeline.

While many patients combine multiple procedures for facial rejuvenation, I’m very selective when combining facelift and rhinoplasty. These are two major surgeries—and I consider factors like complexity, length of surgery, and downtime.

🎯 My Recommendation for Larry:
After reviewing his goals, anatomy, and lifestyle, I recommended a staged plan:

🥇 Phase 1: Rhinoplasty
We reshaped the nasal tip and bridge to better fit his features without impacting breathing. His thick skin and vascularity limited how slim the tip could get, but the change still looked natural and impactful.

🥇 Phase 2: Deep Plane Facelift + Neck Lift
His laxity extended from the neck into the lower face, so I lifted both areas in one surgery - muscle, fat and skin.

🪡 My Technique:
    •    Lifted muscle and fat layers (not just skin) for strong neck and jawline definition
    •    Removed excess skin
    •    Hid scars around the ear and hairline
We also discussed the possibility of posterior hair follicle shifts and electrolysis later if needed.

🥇 Phase 3: Lower Blepharoplasty with Fat Transposition
Larry wanted to address under-eye bags early. I removed loose skin and repositioned fat pads. We held off on upper lids to avoid an overdone look.

👀 Lower Eyelid Surgery (Blepharoplasty) – She Was Tired of Looking Tired 💫

This beautiful patient of mine came to me after having previous lower eyelid surgery and fat grafting around her eyes elsewhere 3 years ago. Since then, she’s still dealing with persistent undereye bags and excess fat grafting that cause some irregularity. She wants to know what can be done to improve the area. She’s never used filler around her eyes, but has had Botox. What’s bothering her now is still the visible bulging under and puffiness under her eyes that wasn’t fully addressed during her first surgery.

✅ What I Recommended:

✔️ Lower Blepharoplasty with Fat Transposition (Revision) – I’ll reposition her fat pads under the eyes to better fill the tear trough and create a smoother eyelid–cheek junction.
✔️ Excision of Excess Fat (Lower Left Eyelid) – On the left side, there’s excess fat that needs to be carefully removed through an external approach. I’ll place the incision in a natural crease that she already has to hide the scar as best as possible.

📚 Education Moment:
Her secondary lower eyelid surgery was more complex. Because she’s had previous surgery, I explained that she might possibly have more prolonged healing. I also explained to her that the discoloration and redness she’s noticing are likely due to increased vascularization from her first surgery—and while I can address the eyebags and volume, pigmentation may need additional treatment down the line with a different procedure.

💡 Fat Transposition is also different from Fat Transfer. Sometimes I recommend both, and my recommendation varies with every patient. Both results take time to fully settle
