When doing resistance exercise, training too lightly won't create adaptation. Training too intensely may result in injury or over-training. The key is reaching "functional failure". When performing a set of an exercise, don't go until you can't do any more. Go until you can't do any more WITH the desired technique. In other words, stop once your form begins to break down. That could be having to alter the technique, shifting your weight unevenly, holding your breath (when not training at or near 1RM loads), etc.
