By being a Patient-Centered Medical Home, recognized by
a national organization called the NCQA, we are held to a higher standard to provide you with health care services that follow your health goals and your needs. We ask you for a partnership with your health care provider to promote prevention, evidence-based medicine, management of your health needs together. We continue to care for you outside of our office as our Care Coordinator and Referral Coordinator faciliate strong communications and documentation with any of your specialists and hospitals you may visit. It is part of their role to advocate for your wellbeing with other healthcare providers and offices. Westminster Medical Clinic additionally offers group visits for patients suffering from chronic disease and/or patients who know someone suffering with illness. Easy communication with us is a must! You can access us via email and telephone. Your lab and diagnostic results are accessible to you on our Patient Portal. In fact, you can also schedule and cancel appointments through the Portal! that is why we are recognized nationally for being "patient-centered"!
