Published in the early 1990s, I read this as a fledgling clinician fascinated by the concept of living long and well. Balancing austerity with lenience, avoidance with curiosity, and necessity with indulgence.

Longevity is currently a favorite buzzword among influencers, faux-gurus, and genuine experts. Like most things, what’s old is new again. Names and technologies change, concepts remain the same.

Our health decisions need not be oppressive… but nurture the very goal of living both long and well which enables more enjoyment of the things we love. Health is freedom.

Humans have always eaten what is available.

From primitive hunter-gatherer societies gathering tubers and hunting small game to the first farmers cultivating local crops to our current on-demand abundance. Human diets have differed based on regional climates, domesticated plants/animals, and migration.

The notion that our ancestors ate “one certain way” is misguided. They ate whatever helped survival.

I am both grateful and proud to be in practice for nearly 25 years. I have cared for people from 52 different countries… members of state/congress, media executives, actors, producers, Olympic and pro level athletes, leaders of industry and also people from underserved communities.

One thing is certain: disease, disorder, and injury does not discriminate. Suffering, while relative to each individual, is universally experienced. Our job is to treat and guide to relieve suffering.

Case: 17yo male, cognitive/emotional symptom dominance.

Concussion symptoms often overlap between physical, cognitive, and emotional.

Comprehensive concussion care should include a mental health component as part of assessment and treatment. A subset of patients may be susceptible to persistent concussion symptoms. Often these patients have pre-existing mental health conditions. Furthermore, injuries can be a catalyst for mental health issues due to poor coping skills, prolonged healing time, and most severely, chronic pathology from repetitive injuries.

Specific clinical examinations and individualized treatments including education, reinforcement, and guidance address mental health components. Thus, patients outcomes are more successful with a decreased risk of prolonged mental health symptoms.

A brief look at some novel strategies involved in brain injury rehabilitation.

1. Markus had a brain vessel hemorrhage as a teenager (for the pros: caused by an AVM) which resulted in a severe brain injury.

2. He has undergone many years of supervised and home (family) guided therapy.

3. I recently was introduced to Markus and my initial goals are to help him with eye-head-neck control.
Humans develop basic postural control first by perceiving our environment through vision, then neck control and finally with full trunk and extremity control. This is the approach you see with some laser feedback. Markus can perceive but has severe challenges with processing and movement.

4. Markus has some control of his right arm but very little activation of his left. We want his brain to begin to recognize his left arm in order to reconnect neural networks. Electrodes contract his left arm and he is instructed to grab his left arm when he feels it move. This was successful. We need many repetitions.

5. Severe brain injuries, strokes, and other neurological disabilities are physically, emotionally, and financially challenging for patients and their families. Most people are not aware of the difficulties and obstacles involved.
