We’re excited to partner with UCHealth Aspen Club Senior Services to bring an important community education event to Fort Collins!

The GUIDE Model, a CMS Innovation Program, is designed to better support individuals living with dementia and the families who care for them. Through care navigation, education, emotional support, and respite resources, this program helps caregivers feel less alone while ensuring patients receive coordinated, thoughtful care.

Thank you to Aspen Club for recognizing the value of this program and to MorningStar Senior Living – Old Town for hosting this event for our community.

We’re honored to partner with Aspens at Fort Collins for their Cognitive Collective — a collaborative group created to empower minds through community, education, and compassionate support for those navigating memory care journeys.

Kate Spruiell, Director of Business Development with Bloom Healthcare, will be presenting on February 25th, sharing how our primary care and hospice teams support individuals in aging safely at home and in community.

The Bloom • Flourish • Thrive Annual Golf Tournament is returning in 2026, bringing together our healthcare and senior care community to support older adults with their essentials - wherever they call home.

Proceeds from this event support seniors living in independent living, assisted living, memory care communities, and private homes, helping provide essential items that promote dignity, comfort, and independence.

We are currently finalizing sponsorship opportunities and registration details and will share more soon. For now, please SAVE THE DATE and plan to join us for a meaningful day on the course in support of seniors across our communities.
