“My baby is 6 months old and has severe eczema on his face. He has been exclusively breastfed, but we are thinking to switching to formula for a few different reasons. In your book it mentions it is not necessary to use a hydrolyzed formula (for allergy prevention), however, do you think it’s better in our case because of his eczema? We have not introduced cow’s milk yet, so I wanted to get your input.”

Great question! I can’t offer individualized guidance here (always talk with your child’s HCP for that), but what I can do is tell you what the general recommendations are. And it can def be confusing because in the past, hydrolyzed formulas were recommended as a way to help reduce allergy risk.

BUT, newer research (including a systematic review and meta-analysis with more than 19K participants) has not found consistent evidence supporting their role (neither partially nor extensively hydrolyzed) in allergy prevention. PMID: 36235669

Generally speaking, rather than avoid them, we want to get common allergens (including cow milk proteins) into baby’s diet early (starting at around 6 months, maybe a bit earlier, but not before 4 months) and consistently going forward, as long as they are tolerated.

Tomorrow I’ll post about choosing the best yogurts for babies in case you prefer to go that route as a way to introduce dairy.

If a baby has already been diagnosed with milk protein allergy (CMA), however, that’s a whole other story!

Our Story

Malkani is a Registered Dietitian Nutritionist, Media Spokesperson for the Academy of Nutrition and Dietetics, and founder of Malina Malkani, LLC (www.MalinaMalkani.com), a nutrition lifestyle company dedicated to providing modern moms with tools, recipes, and programs that make it easier to feed their families a mostly plant based, nutrient-dense, whole food diet (that they really like). She is also the author of Simple and Safe Baby-Led Weaning: How to Integrate Foods, Master Portion Sizes, and Identify Allergies, published in January, 2020. Malina has earned the Commission on Dietetic Registration’s two certificates of training in adult weight management, and completed her dietetic internship at the Bronx VA Hospital, where she subsequently worked as an outpatient weight loss and bariatric surgery dietitian. She graduated from Northwestern University and earned a master’s degree in Clinical Nutrition from New York University. Malina is a mom to three young children and currently lives in Westchester County, New York.
