Nail Canges seen with Coxsackie Virus.

Onychomadesis is a phenomenon in which the nail plate separates from the nail bed because of cessation of activity in the nail matrix, usually followed by incomplete shedding of the nail. Onychomadesis can develop 28 to 56 days after hand-foot-and-mouth disease, which is most often caused by Coxsackie virus infection. Changes in the nails are not permanent, and normal nail growth typically occurs within one to four months.

FUN FACT 👣 This season a lot of feet are going to be gathered in one place 🕎 🧦 🎄

I want to encourage you to ask a question and take off the shoes and socks of your elderly parents, aunts, uncles and friends and look at their skin and nails OF THEIR FEET! (nails, toes, between the toes, top of feet and soles)

Feet are the least looked at on the body.

This is the foot of an elderly Afro Caribbean woman in her eighties who came to me last week for footcare. On inspection I observed an irregular lesion on her foot. It was irregularly shaped, blurred boarders, flat and black. It was not painful. It was not itching. Her daughter never noticed it. She was referred to dermatology, a biopsy was done and a diagnosis of melanoma came back in 24 hours. A treatment plan has been initiated by the doctor.

Foot melanoma — the skin cancer that killed reggae star Bob Marley — is real. And because you're not looking for it, most people notice it too late, when it has already spread to other organs.

Foot melanoma can occur anywhere on the foot, including the top and bottom, between the toes, and even under a toenail. It can arise from an existing mole or from clear skin. It can strike any age group and is equally common in African Americans and Caucasians. For people of African or Asian descent, the feet and hands are the most common places for melanoma to develop.

Nail Debridement

What is under there?

keratin debris — Nail keratin debris results from a fungal infection of the nail. In medical terms this is called onychomycosis or tinea unguium.

The fungal infection breaks down the keratin in the nail to form a white or yellow chalky substance under the nail plate.As keratin debris spreads under the nail, the nail plate typically becomes lifted up off of the nail bed. This separation creates a warm, moist pocket, which is even more conducive to infection. The nail itself will then take on a white or yellowish color.

Nails need to have scheduled debridement if nail treatment is to be effective.

Yesterday I was the recipient of the 2022 Dr. Harold Keltz Distinguished Public Health Service Award from the Westchester County Board of Health.

The Dr. Harold Keltz Distinguished Public Health Service Award is presented annually to a person or community-based organization, whose efforts have made an extraordinary contribution to the public health of Westchester residents but who is not professionally engaged in public health work.

I was recognized for my contribution in foot care to vulnerable populations in my community during the pandemic.

It was an honor, yet humbling, to be amongst other recipients who were innovative in their contributions and pursued compassion, and kindness during such a challenging time.

I earnestly believe that feet tell a story. This is one of many. I am grateful to those who supported my efforts and most importantly to those who allowed me to touch their feet and hear their stories.

Let me use this time to encourage you to examine the feet of seniors and other vulnerable populations who self isolated during the pandemic. The visit may start with feet but you soon realize the needs include conversation, food, a prescription, depression, the need to schedule a doctor's appointment. They need to feel confident, safe and to be able to go about with whatever brings them happiness.

Foot care is collaborative. We are truly in this together. Reach out and donate foot care items such as shoes, socks, moisturizers, foot sprays to shelters in your community, police departments and senior centers. Ask a question and find out their needs.. They often encounter and interact with this population more than you can imagine.
