💙 This is what New York Connects does—every single day 💙

Navigating health care, benefits, and long-term services can feel overwhelming. That’s where our New York Connects team steps in—to guide, advocate, and support individuals, families, and professionals across our community.

Here are just a few examples of how we’ve been helping people recently:
✅ Medicaid Applications & Benefits Navigation
We helped a granddaughter figure out how to apply for Medicaid for her grandmother, explained the required documents, shared online and phone options, and connected her to tools that could unlock additional benefits.

♿ Advocating for Unsafe Medical Equipment
When a resident faced repeated problems with a wheelchair provider—putting his safety at risk—we assisted with filing complaints, drafting a grievance letter, requesting a vendor change, and connecting him to county, Medicare, and NYS Attorney General resources.

🏠 In-Home Care & Long-Term Services
From social workers unsure if their patients qualify for Medicaid, to individuals wanting home health care, we walked callers step-by-step through:
Medicaid eligibility
Managed Long Term Care (MLTC)
In-home services
Care assessments and plan selection

🦻 Access to Assistive Devices
We helped secure free or low-cost hearing aids for someone with a rare disease by connecting them to charitable programs and assisting with applications.

🧠 Urgent Dementia & Hospital Discharge Support
When a family reached out after a loved one with advanced dementia was discharged from the hospital without services, we guided them through:
Fast-tracked Medicaid options
Immediate-need documentation
Dementia-specific eligibility pathways
Home care programs and caregiver protections

🏗️ Home Accessibility & Modifications
We connected a White Plains resident to funding resources for wheelchair-accessible home modifications, including ramps and widened doorways.

✨ And we don’t just give phone numbers—we help you use them.
We offer:
One-on-one guidance
Help with completing applications
Advocacy and follow-up
Three-way calls with agencies
Ongoing support every step of the way

If you or someone you care for needs help with Medicaid, home care, aging services, disability supports, or caregiver resources, New York Connects is here.
