Trempealeau County Health Care Center is a county-owned facility nested in western Wisconsin with roots beginning in 1898. Using a recovery and person-directed care approach the Trempealeau County Health Care Center, focuses on assisting persons to be prepare for community living by promoting the individuals well-being. Pigeon Falls Health Care Center is our nursing home facility that offers full time care, an Adult day program, and Country Ridge Apartments. This proud community facility fills the nursing care needs the Pigeon Falls community asks for. West Central Wisconsin Behavioral Health Clinic is a safe space for people suffering from a multitude of mental illness to be assessed by one of our four professionals. Our areas of assessment for adults include: depression, anxiety disorder, substance abuse, dementia, schizophrenia, marital issues and parenting problems. Responsibilities
• Assist with activities and daily living needs of our clients
• Document and charting as required
• Complete vitals as client care plans require
• Assist with mealtime responsibilities
• Keep the living area safe and clean for clients
• Assist with client transfer needs as requested
• Follow all care plans such as doctor’s orders, treatment plans, or other instructions provided
• Help with medication as directed by licensed practitioners

Mental Health Setting

**20.00 – 21.50 per hour**

We have an opening on our mental health units for a CNA 6 (12) hour shifts and 1 (8) hour shift per pay period. We offer excellent benefits and state retirement. Opportunities to grow, and we support educational assistance.

Responsibilities
• Reports any medical/psychiatric issues to the nurse promptly
• Perform ADLs (activities of daily living) for residents
• Reads and follows individual care plan and/or treatment plans
• Acts as a role model for residents in appearance, behaviors, and interactions
• Meets residents' needs promptly and completes assignments within the shift
• Responds and assists in the management of verbally and/or physically aggressive residents as directed by the nurse
• Assist residents in maintaining a safe, clean environment
• Completes documentation and charting requirements, including but not limited to 15-minute checks, assignment sheets, and weight checks
• Answers signal lights, calls, or alarms to determine residents’ needs
• Bathes, dresses, and undresses residents, as needed
• Serves and collects food trays and feeds residents requiring feeding assistance
• Other duties as assigned

routines and guiding them with games, walks, holiday activities, and other fun tasks.

** $18.50 – $19.42/hr.

Full-time Opening at the main facility (12p – 8p)

Responsibilities
Plan group activities and assist residents with participation.
Assist with mealtime prep, delivery, and clean-up.
Help oversee internal and external outings with residents.
Document resident attendance to scheduled activities and chart as requested.
Be aware of changing behaviors by residents and report to supervision.

Skills
Attend required computer training related to the job.
Follow all Federal, State, and County laws as they apply to your position.
Meet all policies and procedures as stated by the Trempealeau Co Health Care Center.

On Thursday, July 24th we hosted our annual Brownie and Ice Cream Social here at Pigeon Falls Health Care center. Wally Tranberg and Friends was here with live entertainment, pulled pork dinners were served by our Citizen Advisory Board. Thank you to everyone who joined and supported our event.
