March is National Social Work Month, and at TAHC&H we’re proud to recognize the social workers who strengthen care at home across Texas—especially those serving patients and families through home health, hospice, and community-based services.socialwork2_3246406.png
Social workers are often the bridge between clinical care and real-life needs. They help families navigate complex decisions, connect patients to resources and benefits, support caregivers under stress, and ensure the patient’s voice stays at the center of the care plan. In hospice, they also play a critical role in emotional and grief support—providing steady guidance during some of life’s most challenging moments.

Their impact is felt every day in practical, meaningful ways: resolving barriers to safe care, coordinating support services, advocating for patients, and offering compassion when it’s needed most.

To every social worker serving Texans in their homes—thank you for your expertise, your advocacy, and the heart you bring to this work. Your presence makes care at home stronger, more connected, and more human. Learn more from the National Association of Social Workers (NASW)!
