"Ascension Living HOPE is a community-based program whose purpose is to serve the residents of Sedgwick County, KS age 55 and up with a healthcare model including senior home care, adult daycare and specialized geriatric medical care. PACE is a coordinated health and social services program that enables participants to remain in their own homes and communities. It is a unique system of managed care which includes services in the home, at the PACE center, and if necessary, in the hospital or other medical facility. PACE serves the most frail older adults with top quality healthcare and helps people age with dignity at home.

Courtney is a vital part of the IDT (Interdisciplinary Team), where every voice matters and every plan is made with love and intention.

Through IDT planning, Courtney and her fellow team members — from nurses to social workers to therapists — come together to create care plans that are as unique as the individuals we serve. It’s not just about treating symptoms — it’s about understanding lives, goals, and stories.
