Dear Patients,

I am writing this with a heavy heart to inform you that I have made the decision to retire. It has been a great pleasure providing for your health care needs over the years, and it is not easy for me to give it up. Unfortunately, I won’t be able to return from my medical leave and will be closing my clinic.

Please call our office to have your medical records transferred.
