Scoliosis fusions have been around for over 50 years. In that 50 years, the surgery has become more complicated and larger. Overall complication rates are significant. Why not follow the new technology of ultra minimally invasive endoscopic surgery? A number of these individuals with scoliosis can have their problem solved with high-tech ultra, minimally, invasive endoscopy, and no rods, no screws and no cages.

One of the major reasons we started to use hardware 40+ years ago is that the pseudoarthrosis rate was extremely high and was significantly improved with hardware.

You don’t have to be the first to adopt a new technology. However, you don’t want to be the last. IMO total disc replacements and spinal endoscopy are excellent technologies to add for our patients.

In my opinion, modern technology allows a continuum of treatment. First, is the patient a candidate for an endoscopic decompression?
Second, is a patient a candidate for a total disc replacement? Third is the patient going to have to undergo fusion and forget number one and number two?
