Microphlebectomy or Sclerotherapy ?

The decision to use one technique or the other or both depends on the size of the veins and on examination and venous ultrasound imaging as well as vein mapping. This is done at consultation by Dr Karamanoukian.

The images below are from our practice.

Dr. Hratch Karamanoukian is board certified in surgery (American Board of Surgery), thoracic surgery (American Board of Thoracic Surgery)
and Certified by the American Board of Venous and Lymphatic Medicine (ABVLM). He has ultrasound certifications for RVT, RPhS and RPVI. He is a Senior Member of the American Vein and Lymphatic Society. He is a Fellow of the Society of Vascular Medicine (FSVM) and Fellow of the American College of Surgeons (FACS).

Here are some important things you need to know before you get sclerotherapy for spider veins and varicose veins :

1. Sclerotherapy is not an instant fix.
2. It can take 2-3 months or more for the veins to disappear.
3. More than one session may be necessary for a treated or injected area.
4. The injected veins will look worse before they get better.
5. Patients with darker skin pigmentation are at higher risk for permanent skin discoloration.
6. You should be willing to wear compression stockings to optimize post sclerotherapy appearance.
7. You should participate in the topical regime we recommend to optimize healing and reduce skin discoloration.

Make sure you get treated at a vein center by experienced specialists.

Foam Sclerotherapy video for spider veins on the foot by

In this video, Polidochanol in a microfoamed suspension is being injected. This is NOT the hypertonic saline injection that your grandmother got in 1984 !

You may need more than one session of therapy. Some individuals need additional cosmetic laser therapy for the telangiectasias.

Make sure you get treated at a vein center by experienced specialists.

The video shows sclerotherapy for veins that have bled through the skin several times.

Needle sticks are not fun but these are the tiniest needles one can use, which is 30 Guage needles. As well, topical anesthesia is used to make it as comfortable as possible.

Why don’t we post more sclerotherapy videos ?

We try not to do that in the summer because Fall, Winter and Spring are the best times of the year to get cosmetic sclerotherapy.

What is being injected ? Microfoamed sclerosant solution containing Polidochanol. This is not the hypertonic saline solution that your grandmother got in the 1970’s. This solution has anesthetic properties and DOES NOT cause a burning sensation when injected.

If you are interested in being evaluated, call and set up an appointment. We are booking sclerotherapy appointments for September, October, November and December. These procedures are NOT covered by medical insurance. We do not accept patients from outside of our geographic area because we cannot treat everyone out there.

This is NOT the type of sclerotherapy your grandmother got in the 1980s ! What was popular then was using hypertonic saline, which is very painful.

A lot of progress has been made in the types of sclerosant solutions that we use to treat spider veins, reticular veins and varicose veins.

The techniques shown below is with Polidochanol which is pInless as it goes through the veins. It actually has anesthetic properties.

Of course, needle sticks are not fun but these are the tiniest needles one can use, which is 30 Guage needles. As well, topical anesthesia is used to make it as comfortable as possible.

This is NOT hypertonic saline which is painful to inject.

This technique uses STS or Polidochanol which are painless as they go through the veins.

Look at those giant incisions a vascular surgeon used to strip a vein from this patient’s leg 40 years ago !

Our procedures today use tiny puncture wounds. See image on the right.
