The Vein Treatment Center is a National Center of Excellence for Vein Disorders. Hratch Karamanoukian, MD FACS and Raffy Karamanoukian, MD FACS are 2 of 4 Board Certified Phlebologists (vein specialists) in the Buffalo-Niagara frontier. A phlebologist has added Board Certification from the American Board of Venous and Lymphatic Medicine (ABVLM). Lipodystrophy can also occur in men.

Lipedema, also known as lipodystrophy, is a condition involving abnormal fat deposits often in the legs, can sometimes coexist with venous insufficiency, where veins struggle to return blood to the heart, leading to swelling. Studies indicate that a notable percentage of lipedema patients may also have venous insufficiency, but the exact figure isn’t universally agreed upon.

According to some medical sources, approximately 25% of patients with lipedema also have chronic venous disease. This percentage may increase to 50% for those who also have lymphedema, a condition involving lymph fluid buildup. However, these figures are based on limited sources and may not be definitive, as more comprehensive research is needed.

Urgent care centers and hospitals do not have sonographers on call to check for blood clots in the legs. Instead, they use a D-Dimer test.

D- dimer tests can be falsely elevated in many clinical situations including pregnancy, cancer, recent surgery or trauma, infection, inflammation or rheumatoid arthritis.

In a vein practice, we do it better. We do a Doppler ultrasound scan to exclude DVT or superficial blood clots.

This is a before and after treatment images of a patient who was itching so much that he created bruising in his leg.

A lot of non physicians commented that this was cellulitis or thrombophlebitis or DVT. It was not.

The image below shows the appearance of the leg 5 days after treatment for venous insufficiency.

Are the veins on the outer thigh different than the veins that originate from saphenous veins ?

Short Answer : YES.

Varicose veins originating in the lateral thigh typically arise from non-saphenous sources. Unlike varicose veins associated with the great or small saphenous veins, those in the lateral thigh often stem from tributaries or perforating veins not directly connected to the saphenous system. Common non-saphenous sources include:

Lateral subdermic venous plexus: A network of superficial veins in the lateral thigh.

Perforating veins: Veins connecting the superficial to deep venous systems, such as those in the thigh region.

Pelvic vein incompetence: Reflux from pelvic veins can contribute to lateral thigh varicosities.

These veins may be linked to conditions like venous insufficiency or reflux in non-saphenous systems, often requiring ultrasound evaluation for accurate diagnosis and treatment planning.

This is NOT hypertonic saline which is painful to inject.

This technique uses STS or Polidochanol which are painless as they go through the veins.

Of course, needle sticks are not fun but these are the tiniest needles one can use, which is 30 Guage needles. As well, topical anesthesia is used to make it as comfortable as possible.
