MCHC Welcomes Dentist Dr. Caleb Pham

MCHC Health Centers is pleased to welcome Caleb Pham, DMD, who will provide dental care for children and adults at Little Lake Health Center in Willits.

“As a child, I remember having a bad toothache. We couldn’t go to the dentist because my family couldn’t afford it,” he recalls.

Pham appreciates MCHC’s mission of providing healthcare to everyone, regardless of their ability to pay or their insurance status, aligns with his personal values.

MCHC seeks a Quality Improvement Program Manager to develop and execute strategies enhancing our quality performance. This key role will oversee projects and initiatives for our Quality Improvement Program, acting as a subject matter expert. In this collaborative position, you will work with internal teams, external vendors, and healthcare providers to meet our goals. You’ll manage a team of care coordinators who handle outreach for Partnership and other patients.

This role requires interpreting complex regulatory documents, leading meetings, and developing the Quality Improvement Transformation Program for MCHC.

The Care Coordinator is responsible for engaging and assisting patients on their journey towards sustainable long-term improvements. He or she will partner with clients seeking self-directed, lasting changes that are aligned with the patients’ values and their plan of care to promote health and wellness. The Care Coordinator will facilitate client education, support groups, and engage targeted clients who may otherwise be lost to care.
