What is paradoxical masseter bulging after neurotoxin treatment of the masseter.
Paradoxical bulging happens when only part of the masseter muscle is effectively weakened, while an untreated or under-treated portion remains active and compensates.

The masseter is anatomically layered (superficial and deep components).
• It has variable fiber orientation and thickness across patients.
• If toxin is placed too superficially, too posteriorly, or too inferiorly, you may:
• Weaken one segment
• Leave adjacent fibers functional
and during clenching, like me the untreated fibers over-contract, creating a visible focal bulge.

What can cause it?
• Uneven injection depth
• Under-dosing in thick hypertrophic muscle
• Treating only the lower third
• Strong anterior fibers near the mandibular body
• Variant anatomy (accessory masseteric fibers)
