In 2024, each of the three counties served by CFHC had its Community Health Needs Assessments (CHNA) updated and the final plans have been made available. These assessments help health leaders evaluate the health and wellness of the community they serve and identify gaps and challenges that should be addressed through new programs, services and policy changes.
