MDS Coordinator/Registered Nurse

We are looking to hire a full-time MDS Coordinator at our 46-bed skilled nursing facility in Winfield, Iowa.

This person will be part of our management team and strong candidates will have:
- An active RN license
- Experience in both SNF and hospital settings
- Experience with the MDS processes
- Strong attention to detail and a learning attitude
- Ability to multi-task and meet deadlines
- Problem solving and critical thinking skills
- Communication skills to lead the interdisciplinary care plan team

Other responsibilities include:
- Coordinating resident and family care plan meetings
- Medicare Coordinator for Part A and B services
- ABN and discharge planning
- and more!

We love celebrating success stories like this one 💙

Meet Mark and his wife, Shelly. In late February, Mark suffered a sudden cardiac arrest — a terrifying moment that could have had a very different outcome. But thanks to Shelly’s knowledge of CPR and her quick, calm response, Mark is still here today.

Shelly performed CPR for an incredible 15 minutes until first responders arrived on the scene — a true act of strength, courage, and love that made all the difference.

Following his hospitalization, Mark and Shelly made the choice to continue his recovery at Sunrise Terrace rather than a rehab hospital, so he could be closer to home.

After spending time with us for recovery and rehabilitation, we are so proud to share that Mark has officially discharged and returned home 🏡

This story is a powerful reminder of just how important CPR knowledge can be. In an emergency, those first few minutes matter — and having someone nearby who knows what to do can truly mean the difference between life and loss.

Mark’s strength and determination, combined with Shelly’s heroic actions, made this outcome possible. We are honored to have been part of his journey and wish them both all the best as they continue forward together.

We had a wonderful time celebrating St. Patrick’s Day with a festive party . 💚 Our residents enjoyed great company, tasty treats, and plenty of Irish cheer.

From smiles to shamrocks, it was a day full of joy and special moments we’ll always treasure. We feel so lucky to share these celebrations together! ☘️

March is Social Week month and we get to celebrate Angie, our incredible Social Worker! 🙌🏻

In a small facility like ours, one person wears many hats, and we are so lucky to have someone who does it all with compassion, patience, and dedication.
