Our APRNs are vital members of our primary care team. Sports Physicals for USD 465 Athletics!

We've lined up options for your sports physicals. Remember to grab your full physical packet from the HS or MS office and fill out your part before your appointment. Providers won't have the paperwork, so be sure to get it from us!

A completed physical must be submitted before participating in practice.
