Who We Are
Founded in 1994, the Consortium for Southeast Healthcare Quality (COSEHQ) is a 501(c)(3) nonprofit corporation. COSEHQ’s Mission is to empower health care professionals, patients, and the public with better knowledge, tools, and competencies through continuous quality improvement to secure cardiometabolic health for all people. COSEHQ’s Board of Directors comprises 30 world leaders representing academia, basic science, private practice physicians, health systems, and the business community. COSEHQ has integrated a network of academic health centers and community primary care practices known as Centers of Excellence ™. Currently there are 23 diverse Centers of Excellence in the Southeast Region and Caribbean. Practices are required to achieve defined benchmarks in the areas of lipids (LDL), blood pressure, and A1c to receive the Centers of Excellence designation. Since 2015, COSEHQ has functioned as a practice transformation network, titled QualityImpact, under a cooperative agreement with the Centers for Medicare and Medicaid Services Transforming Clinical Practices Initiative. COSEHQ is collecting and analyzing clinical data from 4,000 enrolled providers to identify gaps in care. This partnership equips practices with the technology, facilitation and clinical quality improvement expertise to help them thrive in a value-based delivery environment. COSEHQ offers continuing medical education (CME) that initially focused on hypertension and targeted an audience in North Carolina where the Founders resided. New activities have been developed, such as diabetes and other disease management, Sustainable Transformation, Billing and Coding, and other areas. With the increasing offerings of medical education programs, COSEHQ determined that becoming an accredited provider was an important strategy in achieving its mission and goals; the COSEHQ CME Program received initial accreditation from Accreditation Council for Continuing Medical Education (ACCME) in 2003 and was recently awarded Accreditation with Accomendation in 2019.
