If you wish to retreive your medical records (paper copy), they will be available for pick up Friday, April 4th, 2025 from 12:00pm to 4:00pm and Tuesday, April 8th, 2025 from 12:00pm-4:00pm. Records not picked up on those dates will be permanently destroyed. (April 8th, 2025)

I.D. and signature are required for release of records.
