Our physician, Dr. Robert Bloomfield, believes in providing comprehensive health care services in a friendly, relaxed atmosphere. We also believe in educating our patients as we work with them to achieve the best physical state possible. Our physician is certified by the American Board of Internal Medicine and comes to our practice with years of experience in his area of specialty. He takes great care to work with each patient on a personal, unhurried manner, in order to provide informed, consistent care at all times, using traditional, complementary and alternative medicine, as appropriate. We spend a great deal of time with each patient; and waiting time is always minimal.

COVID-19 Vaccine info:

CVS and Walgreens are administering the first two COVID-19 vaccines at long-term care facilities.
Healthcare facilities are ramping up vaccination clinics for their workers during this first phase when vaccine supplies are still scarce.
Many pharmacies are planning to make the vaccine available to the public once supplies increase and high-priority groups receive their vaccines first.
Healthcare workers and long-term care residents and staff are preparing for an unprecedented event: the arrival of the COVID-19 vaccine.

Operation Warp Speed (OWS), the federal government’s campaign to speed up vaccine availability, is well underway. The vaccine from pharmaceutical companies Pfizer-BioNTech received emergency use authorization from the Food and Drug Administration (FDA) on December 11, and another vaccine from Moderna got approval a week later.

As we await emergency authorization of more vaccines to help alleviate limited initial supply, 60% of U.S. pharmacies are preparing to administer the COVID-19 vaccine, according to the Department of Health and Human Services (HHS). An estimated 21 million healthcare workers and 3 million long-term care residents will be eligible for the first vaccines, according to the Centers for Disease Control and Prevention (CDC).

Retail pharmacies Walgreens and CVS are providing the first phase of vaccines at long-term care facilities. With cases surging higher than in the spring in some areas in the U.S. and deaths from COVID-19 surpassing 300,000, the vaccine can’t come too soon.

Here’s more information about where the vaccine will be available and who will be eligible for it first.

Where to get the novel coronavirus (COVID-19) vaccine
VaccineFinder.org is a platform that helps people find vaccine providers in their communities. When the COVID-19 vaccine becomes available, VaccineFinder will require providers to report the number of vaccines they have in their inventory each day. Providers can also make their locations visible to the public when the vaccine becomes more widely available.

For now, the vaccine will be available to a limited number of people through pharmacies and healthcare facilities. More information on each is below.

Pharmacies
From independent pharmacies to regional/national chains, drugstores and grocers across the country have partnered with HHS to make the vaccine available. They are starting by administering the vaccine to healthcare workers and residents and employees of long-term care facilities. (See below for all three phases of the vaccine’s initial roll-out.)

Albertsons: Albertsons expects to have the vaccine at some locations for select populations when it first becomes available and then eventually at all locations.
CVS: CVS stores will add the COVID-19 vaccine to its scheduling tool as soon as it’s available. The tool will require you to schedule both the first shot and the follow-up vaccine.
H-E-B: Texas-based H-E-B is prepared to follow the CDC’s vaccine distribution schedule.
Hy-Vee: Hy-Vee plans on hiring 1,000 licensed pharmacy technicians for its 270 pharmacies. The new hires will provide vaccine support to its customers in Iowa, Illinois, Kansas, Minnesota, Missouri, Nebraska, South Dakota, and Wisconsin.
Rite Aid: All Rite Aid locations are staged and ready to administer the vaccine, according to the store’s website.
Target: Target employees and customers will be able to get the vaccine at its in-store pharmacies once it’s made available to the public, according to Target’s website.
Walgreens: Walgreens will support the government’s efforts to deliver vaccines to long-term care facilities. The company will also provide vaccines to consumers at all of its 9,000+ store locations. Visit Walgreens’ COVID-19 FAQs page for more information.

Healthcare facilities
Healthcare workers along with residents and employees of long-term care sites, arguably the most vulnerable populations, can expect to be first in line for the vaccine. Facilities that are ready for vaccinations include:

Brookdale Senior Living: Plans are in place for vaccination clinics at Brookdale’s 700 communities for workers and residents in all levels of care including independent living, assisted living, memory care, and skilled nursing.
Banner Health: Banner Health is ready to distribute vaccines as of Dec. 14, Chief Clinical Officer Marjorie Bessel said. It has staff available to administer the shots first to its frontline workers and then to high-risk populations. Banner Health has facilities in Arizona, California, Colorado, Nebraska, Nevada, and Wyoming.

Dear Patients:
NC Medicaid is reimbursing COVID-19 testing costs for individuals without insurance who enroll in the NC Medicaid Optional COVID-19 Testing (MCV) program.
To be eligible, an individual must:
*Live in North Carolina
*Be a U.S. citizen or U.S. national or have eligible immigration status
*Not be covered by Medicaid, Medicare or other health insurance

NOTE: Individuals currently enrolled in NC Medicaid’s limited “Family Planning Only” benefit and who have no other
health insurance coverage are automatically enrolled in the MCV program and do not need to complete an application.
Individuals who qualify for the MCV program will remain in the program throughout the federal declaration of emergency period. Costs for COVID-19 tests are covered retroactive to June 1, 2020, provided individuals were
uninsured at the time of the test.

Individuals may complete an application online.

If an online application does not work for an individual, complete a paper application and mail it to the address on the

COVID-19 Safety Guidelines
(Adapted by EHP from CDC and other guidelines)
• Call Ahead: Our receptionist, Clark Kellam, will contact you in advance of a visit. He will outline the purpose of the visit and will attempt to ensure that you are aware and available for the visit before entering the office or a telehealth appointment is initiated. Please call us at (336)659-6250 to schedule/reschedule a visit or if you have any questions or concerns.
• The glass door into the waiting room will stay locked, and patients will be ushered in at the time of their appointment. We request that you and any accompanying family member or friend wait in the comfort and safety of your car, until that time. We will temporarily discontinue utilizing the waiting room to avoid risk of exposure.
• Please DO NOT come into the office without calling first, so we can confirm our availability, be ready to receive you, and make sure it is safe for you and our staff to hold an in-office visit.
• If you are experiencing fever, fatigue, phlegm production, runny nose, cough, shortness of breath and/or loss of taste and smell senses, please call one of the few Acute Respiratory clinics (set up to receive COVID-19 symptomatic patients safely (See list below, and take a picture of it with your phone), to be assessed for further care. Please DO NOT come into Evergreen office to be seen in person unannounced. Unfortunately, currently, it is not safe for you and for our staff.
• Temperature Checks: Evergreen Health Promotion staff will take daily temperature checks to support, as much as possible, their health before coming into the office. We request the same from you, our patients. Please measure your temperature, if possible, before you and any accompanying family member or friend enter, to help preserve the safety of all.
• Personal Protection Equipment During Visits: Both Evergreen Health Promotion staff as well as patients are required to wear a face mask properly (covering both nose and mouth) during the entire patient visits in the office. The staff is required to wear gloves as well, and we encourage you to do the same. We will make every effort, considering inventory and accessibility, to provide patients who do not have access to gloves with a pair for the duration of visit. If you do not have access to gloves, and we are unable to provide you with a pair, you will be required to wash your hands upon entering the office and before and after using the bathroom.
• Bathroom Use: Please use a paper towel to flush the commode and handle the doorknob. Also, lower the commode cover before flushing to prevent splatter. Use the sink outside the bathroom to wash your hands (splatter in a small closed spaces proposes a higher risk) We would like to suggest that the safest practice would be to use the bathroom at your home prior to coming in, though we understand this may not always be a practical option.
• At the completion of a visit, the staff will wash their hands and disinfect the exam room, any instruments used, (such as stethoscope), and surfaces such as door handles, desktops, and exam tables. Our staff will also practice social distancing to the best possible degree.
• It is preferable for your healthcare provider to obtain vital signs at the beginning of every encounter. We understand that due to COVID-19 stay-at-home orders, and other reasons, it can become difficult or altogether impossible to collect that data. Therefore, we encourage you, per reasonable budget limitations, to purchase over time, as your budget allows, a scale, a blood pressure cuff (digital varieties are usually affordable to most budgets), a digital pulse/ox measuring device, and a forehead non-tactile thermometer.
• Video Calls: As an alternative to an in-person visit, we offer Telehealth video or phone visits. We will send you, via email or text a video link to conduct a virtual meeting.
• Remember “the 3 W” s:
 Wear a mask (and gloves)
 Wash your hands frequently
 Wait at least 6 ft apart (be generous!) …

Stop the spread of Coronavirus

As the COVID-19 pandemic continues, there are still important ways that you can help stop the spread of COVID-19. The best way to prevent illness is to avoid being exposed, which means to stay home as much as possible. If you must go out, be sure to practice social distancing, leaving 6 feet between you and any person you come in contact with.

Other ways to help prevent the spread of this illness include:

Wash your hands often with soap and water,
Cover your mouth and nose when you cough or sneeze,
Use a face-covering or mask when you go out,
Clean and disinfect frequently touched surfaces daily,
Call your doctor if you think you’re experiencing symptoms.
