Medicare Changes for 2024 - Medicare Part A in 2024 - Beginning next year, the inpatient hospital deductible will increase to $1,632, resulting in an additional $32 out-of-pocket expense before coverage takes effect. Beyond that, daily coinsurance for hospital stays lasting between 61 and 90 days will be $408, and $816 for lifetime reserve days. For those utilizing skilled nursing facilities, the daily coinsurance for days 21 through 100 will be $204. It's crucial for individuals on Medicare to plan ahead and stay informed about these adjustments.

Medicare Part B Premiums: The standard monthly Part B premium is set to increase by almost $10, rising from $164.90 to $174.70. Most Medicare beneficiaries have this premium deducted from their Social Security benefits. In 2024, the average monthly Social Security payment will increase by $59, thanks to a 3.2 percent Social Security cost-of-living increase (COLA). This COLA will more than cover the added Part B monthly premium for most beneficiaries. Additionally, there are adjustments in premiums for high-income beneficiaries. For instance, individuals earning between $103,000 and $129,000 will pay a monthly premium of $274.70, while those earning above $500,000 will pay $515.10. These premiums vary based on income brackets.

Each year, the Centers for Medicare & Medicaid Services (CMS) set the following year’s Part B premium. In 2024, the Part B base premium is $174.70. Most people will pay this amount. A small number of people may pay a premium that is lower than the base premium in 2024. These people are protected by the hold harmless rule.

The hold harmless rule protects you from having your previous year’s Social Security benefit level reduced by an increase in the Part B premium so long as:

You were entitled to Social Security benefits for November and December in 2023;
The Medicare Part B premium will be or was deducted from your Social Security benefits in November 2023 through January 2024;
You do not already pay higher Part B premiums because of Income-Related Monthly Adjustment Amount (IRMAA) eligibility;
And, you do not receive a Cost of Living Adjustment (COLA) large enough to cover the increased premium. COLA is additional income given to Social Security recipients to protect against inflation decreasing the benefit’s purchasing power. The COLA in 2024 is 3.2% of your Social Security benefit.

The COLA in 2024 is likely to cover the full Part B premium for most people. If your COLA is large enough to cover the full amount of your increased premium, you will not be held harmless and your premium will be $174.70.

The hold harmless provision does NOT protect you if:

You are new to Medicare in 2024. Hold harmless does not apply to you because you have not been enrolled in Medicare Part B long enough to qualify.
You are subject to IRMAA.
You are enrolled in a Medicare Savings Program (MSP). However, the MSP should continue paying for your full Part B premium.
You were enrolled in a Medicare Savings Program in 2023 but lost the program because your income increased or you failed to recertify.

Note: If you qualify for the hold harmless provision but pay a Part B late enrollment penalty, the penalty will not be waived, and it may increase. This is because the penalty will be calculated based on the new, higher premium—even if you are not paying that higher amount.

CMS recently announced 2024 monthly premium, deductible and co-pay amounts for Medicare Part A and Part B. The 2024 standard Medicare Part B monthly premium will be $174.70. This is a increase of $9.80 when compared to the 2023 monthly premium of $164.90. This amounts to a 5.9% increase, after a 3.0% decrease last year. The annual Part B deductible for 2024 is $240, up from $226. The Part A deductibles and coinsurances will increase 2.0% for 2024, after an increase of 2.8% in 2023.
